摘要

目的 分析前列腺癌(PCa)组织中miR-125b-5p、miR-1207-3p、miR-802表达水平,并探析指标与病理参数、预后的关系.方法 选取161例PCa患者作为研究对象,病例收集时间为2019年1月至2021年7月,收集地点为宁波市医疗中心李惠利医院.患者从确诊当日起开始随访,随访时间截至2024年7月,随访期间失访3例,最终纳入158例进行研究,根据预后情况分为预后不良组(53例)和预后良好组(105例).收集PCa患者手术切除的新鲜癌组织和癌旁组织标本,比较癌组织和癌旁组织miR-125b-5p、miR-1207-3p、miR-802表达,分析PCa患者癌组织miR-125b-5p、miR-1207-3p、miR-802表达与临床病理特征的关系,比较不同预后PCa患者癌组织miR-125b-5p、miR-1207-3p、miR-802表达.应用Cox回归分析PCa患者不良预后的危险因素,并将预后不良组纳入阳性,预后良好组纳入阴性,分析癌组织miR-125b-5p、miR-1207-3p、miR-802对PCa患者不良预后的预测价值,应用受试者工作特征曲线(ROC)获取曲线下面积(AUC).结果 PCa患者癌组织miR-125b-5p、miR-802表达低于癌旁组织(P<0.05),癌组织miR-1207-3p表达高于癌旁组织(P<0.05).Gleason评分>7分、Ⅲ~Ⅳ期、有淋巴结转移的PCa患者癌组织miR-125b-5p、miR-802表达低于Gleason评分≤7分、Ⅰ~Ⅱ期、无淋巴结转移者(P<0.05);而癌组织miR-1207-3p表达高于Gleason评分≤7分、Ⅰ~Ⅱ期、无淋巴结转移者(P<0.05).预后不良组中癌组织miR-125b-5p、miR-802表达低于预后良好组(P<0.05),癌组织miR-1207-3p表达高于预后良好组(P<0.05).Cox回归分析可见,癌组织miR-125b-5p表达偏低(HR=1.091)、miR-1207-3p表达偏高(HR=1.099)、miR-802表达偏低(HR=1.135)与PCa患者不良预后有关(P<0.05).ROC分析可见,癌组织miR-125b-5p、miR-1207-3p、miR-802联合检测预测PCa患者不良预后的AUC值为0.900,高于三者单一检测(AUC 值分别为 0.689、0.679、0.758,P<0.05).结论 PCa 组织中 miR-125b-5p、miR-802 呈低表达,而miR-1207-3p呈高表达,三者与患者病理、预后有关,且三者联合检测预测PCa患者不良预后更具优势.

Abstract

Objective To analyze the expression levels of miR-125b-5p,miR-1207-3p,and miR-802 in prostate cancer(PCa)tissues,and to explore the relationship between the indicators and pathological parameters and prognosis.Methods A total of 161 PCa patients were selected as the study subjects,and the cases were collected from January 2019 to July 2021 at Ningbo Medical Center Li Huili Hospital.The patients were followed up from the day of diagnosis until July 2024.During the follow-up period,3 cases were lost to follow-up,and 158 cases were finally included for the study.According to the prognosis,they were divided into the poor prognosis group(53 cases)and the good prognosis group(105 cases).Fresh cancer tissues and paracancerous tissue specimens surgically removed from patients with PCa were collected,and the expression of miR-125b-5p,miR-1207-3p,and miR-802 in cancer tissues and paracancerous tissues were compared.The relationship between the expression of miR-125b-5p,miR-1207-3p,and miR-802 in cancer tissues and clinicopathological features of PCa patients was analyzed,and the expression of miR-125b-5p,miR-1207-3p,and miR-802 in cancer tissues of PCa patients with different prognosis were compared.The risk factors for poor prognosis of PCa patients were analyzed by Cox regression,and the poor prognosis group was included in the positive and the good prognosis group was included in the negative.The predictive value of miR-125b-5p,miR-1207-3p,and miR-802 of cancer tissues on the poor prognosis of PCa patients was analyzed.Area under the curve(AUC)was obtained using receiver operating characteristic curve(ROC).Results The expression of miR-125b-5p and miR-802 in cancer tissues of PCa patients was lower than that in paracancerous tissues(P<0.05),and the expression of miR-1207-3p in cancer tissues was higher than that in paracancer tissues(P<0.05).The expression of miR-125b-5p and miR-802 in cancer tissues of PCa patients with Gleason score>7,stage Ⅲ-Ⅳ,and lymph node metastasis was lower than that in those with Gleason score≤7,stage Ⅰ-Ⅱ,and no lymph node metastasis(P<0.05),while the expression of miR-1207-3p in cancer tissues was higher than that in those with Gleason score≤7,stage Ⅰ-Ⅱ,and no lymph node metastasis(P<0.05).The expression of miR-125b-5p and miR-802 in cancer tissues of the poor prognosis group was lower than that in the good prognosis group(P<0.05),and the expression of miR-1207-3p in cancer tissues was higher than that in the good prognosis group(P<0.05).Cox regression analysis showed that low expression of miR-125b-5p(HR=1.091),high expression of miR-1207-3p(HR=1.099),and low expression of miR-802(HR=1.135)in cancer tissues were associated with poor prognosis of patients with PCa(P<0.05).ROC analysis showed that the AUC value of the combined detection of miR-125b-5p,miR-1207-3p,and miR-802 in cancer tissues for predicting poor prognosis in patients with PCa was 0.900,which was higher than that of the three single detections(AUC values were 0.689,0.679,and 0.758,respectively,P<0.05).Conclusion MiR-125b-5p and miR-802 are lowly expressed in PCa tissues,while miR-1207-3p is highly expressed.The three are related to the pathological parameters and prognosis of patients,and the combined detection of the three was more advantageous in predicting the poor prognosis of patients with PCa.
