摘要

脑深部电刺激(DBS)是对关键大脑结构进行电刺激来治疗神经精神疾病的方法.DBS疗法的起源可以追溯到立体定向功能神经外科早期,在神经核团毁损术中常规进行的用于确定靶点的电刺激测试.部分学者还通过颅内电极外置,在术后几周或几个月内为患者进行间歇性电刺激治疗.现代DBS的发展始于BENABID团队在1987年发表的关于丘脑电刺激治疗震颤的文章.DBS硬件系统由颅内电极、延伸导线和植入式脉冲发生器组成.目前,DBS疗法对于运动障碍疾病,如帕金森病、特发性震颤和肌张力障碍,已经是成熟的外科治疗手段.另外该疗法也被积极地用于大脑环路病变导致的其他疾病,包括难治性精神疾病和癫痫等.即将到来的电极设计、刺激范式、闭环和感知技术方面的突破有望提高DBS的疗效和安全性.

Abstract

Deep brain stimulation(DBS)is an effective treatment for various neurological and psychiatric conditions through delivery of electrical stimulation to key brain structures.The origin of DBS can be traced back to the practice of intra-operative electrical stimulation,used for target exploration prior to lesioning,during the early years of stereotactic functional neurosurgery.Externalized electrodes were implanted and used for intermittent electrical stimulation during weeks or months prior to subsequent ablation,as the implantable neurostimulator was unavailable.It is generally acknowledged that the development of modern DBS started with an article published in 1987 by BENABID's team on thalamic DBS for tremor.The DBS hardware system consists of an intracranial electrode,an extension wire and an implantable pulse generator.Nowadays,DBS is a well-established surgical treatment in movement disorders such as Parkinson's disease,essential tremor and dystonia,and is also under active investigation for other conditions linked to pathological brain circuitry,including refractory psychiatric disorders and epilepsy.Oncoming breakthroughs in electrode designs,stimulation paradigms,closed-loop and sensing technologies are expected to enhance the efficacy and safety of DBS.
