早期康复活动对重症监护病房呼吸衰竭患者康复的影响

目的 探讨早期康复活动对重症监护病房(ICU)呼吸衰竭患者康复的影响情况。方法 分析咸阳市第一人民医院2014年1月～2016年1月收治的呼吸衰竭患者150例的临床资料，依据康复护理方式不同进行分组，分为常规康复组50例和早期康复活动组100例。观察两组呼吸衰竭患者康复前后呼吸频率、心率、氧分压、二氧化碳分压、1s用力呼吸容积(FEV1)、FEV1/用力肺活量(FVC)、FEV1占预计值的百分比(FEV1％)情况，观察两组呼吸衰竭患者机械通气时间、ICU治疗天数、总住院天数及呼吸机相关性并发症发生率。结果 两组呼吸衰竭患者 康复前呼吸频率、心率、氧分压、二氧化碳分压、FEV1、FEV1/FVC、FEV1％比较，差异无统计学意义(P＞0。05)，康复后常规康复组和早期康复活动组呼吸频率、心率、二氧化碳分压均低于康复前，氧分压、FEV1、FEV1/FVC、FEV1％均高于康复前，早期康复活动组患者康复后呼吸频率、心率、二氧化碳分压均低于常规康复组，氧分压、FEV1、FEV1/FVC、FEV1％均高于常规康复组，早期康复活动组患者机械通气时间、ICU治疗天数、总住院天数均低于常规康复组，呼吸机相关性并发症发生率低于常规康复组，差异均有统计学意义(P＜0。05)。结论 早期康复活动在呼吸衰竭患者中应用，可以改善患者血氧分压和肺功能，缩短治疗时间，降低并发症，值得临床推广应用。
Influence of recovery for respiratory failure patients in the ICU by early rehabilitation activities
Objective To approach influence of recovery for respiratory failure patients in the ICU by early rehabilitation activities.Methods The 150 cases clinical data of respiratory failure patients in Xianyang No.1 People's Hospital from January 2014 to January2016 were analyzed,which were divided into two groups by different rehabilitation nursing ways:conventional rehabilitation group 50 cases and early rehabilitation activities 100 cases.The RR,HR,PaO2,PaCO2,FEV1,FEV1/FVC,FEV1％ of two groups respiratory failure patients before rehabilitation and after rehabilitation were detected,the mechanical ventilation time,ICU treatment days,total HLOS and complication rate of breathing machine correlation for two groups respiratory failure patients were detected.Results The RR,HR,PaO2,PaCO2,FEV1,FEV1/FVC,FEV1％,FEV1％ of two groups respiratory failure patients before rehabilitation were no difference (P ＞ 0.05),the RR,HR,PaCO2 of conventional rehabilitation group and early rehabilitation activities group after rehabilitation were lower than before rehabilitation,the PaO2,FEV1,FEV1/FVC,FEV1％ were higher than before rehabilitation,the RR,HR,PaCO2 of early rehabilitation activities group after rehabilitation were lower than conventional rehabilitation group,the PaO2,FEV1,FEV1/FVC,FEV1％ were higher than conventional rehabilitation group,the mechanical ventilation time mechanical ventilation,ICU treatment days of early rehabilitation activities group patients were lower than conventional rehabilitation group,complication rate of breathing machine correlation were lower than conventional rehabilitation group,the difference were statistical significanct (P ＜ 0.05).Conclusion The application of respiratory failure patients by early rehabilitation activities can improve PaO2 and lung function of patients,shorten treatment time,decrease complication,which is worthy to be used.
