矮小症儿童血尿酸与体格发育指标、胰岛素样生长因子1及丙氨酸转氨酶水平的关系

目的 探索矮小症(SS)儿童血尿酸(UA)与体格发育指标、胰岛素样生长因子1(IGF-1)及丙氨酸转氨酶(ALT)水平的关系.方法 回顾性分析2021年1月至12月浙江省舟山市妇女儿童医院164例SS儿童(观察组)及162名健康儿童(对照组)的临床资料.比较两组体格发育指标及UA、IGF-1、ALT水平;SS的影响因素采用logistic回归分析;UA与体格发育指标、IGF-1、ALT的相关性采用Pearson相关系数和分层回归模型分析.结果 观察组UA、ALT、体重、体重指数(BMI)高于对照组,IGF-1、身高低于对照组,差异有统计学意义(P<0.05).UA、IGF-1、ALT、身高、体重、BMI 是 SS 的影响因素(P<0.05).UA 与 IGF-1、身高呈负相关(r=-0.294、-0.371,P<0.01),与ALT、体重、BMI呈正相关(r=0.362、0.291、0.223,P<0.01).分层回归分析显示,IGF-1对UA产生显著的负向影响;加入ALT后,对UA产生显著的正向影响;加入身高后,对UA产生显著的负向影响;加入体重后,对UA产生显著的正向影响;加入BMI后,对UA产生显著的正向影响.结论 SS儿童存在体格改变,UA、ALT、体重、BMI升高,IGF-1、身高降低,且UA与体格发育指标、IGF-1、ALT水平存在一定相关性.
Relationship between serum uric acid and physical development indexes,insulin-like growth factor-1,and alanine aminotransferase levels in chil-dren with short stature
Objective To explore the relationship between blood uric acid(UA)and physical development indexes,insulin-like growth factor-1(IGF-1),and alanine transaminotransferase(ALT)levels in children with short stature(SS).Methods The clinical data of 164 SS children(observation group)and 162 healthy children(control group)in Zhoushan Women's and Children's Hospital of Zhejiang Province from January to December 2021 were retrospectively analyzed.Physical development indexes,UA,IGF-1,and ALT levels were compared between two groups;logistic regression analysis was used to analyze the influencing factors of SS;the relationship between UA and physical development indexes,IGF-1 and ALT was analyzed by Pearson correlation coefficient and hierarchical regression model.Results UA,ALT,weight,and body mass index(BMI)in observation group were higher than those in control group,IGF-1 and height were lower than those in control group,and the differences were statistically significant(P<0.05).UA,IGF-1,ALT,height,weight,and BMI were the influencing factors of SS(P<0.05).UA was negatively correlated with IGF-1 and height(r=-0.294,-0.371,P<0.01),and positively correlated with ALT,weight,and BMI(r=0.362,0.291,0.223,P<0.01).Hierarchical regression analysis showed that IGF-1 had a significant negative effect on UA;when ALT was added,it had a significant positive effect on UA;when height was added,it had a significant negative effect on UA;when weight was added,it had a significant positive effect on UA;when weight was added,it had a significant positive effect on UA.Conclusion Children with SS have physical changes,UA,ALT,weight,and BMI increased,IGF-1 and height decreased,and UA is correlated with physical development indexes,IGF-1,and ALT levels.
