目的 研究β内啡肽(β-EP)、环氧化酶-2(COX-2)、多巴胺受体D2(DRD2)在右向左分流(RLS)合并有先兆偏头痛患者血清中的表达及其与焦虑状况的关联性.方法 选取 2022 年 7 月至 2023 年 7 月吉林省一汽总医院收治的214例检查存在RLS的有先兆偏头痛患者,记作RLS偏头痛组,50例检查无RLS的有先兆偏头痛患者作为对照组,比较两组血清β-EP、COX-2、DRD2 水平及焦虑自评量表(SAS)评分.另将RLS偏头痛组患者根据检查结果分为少、中、大量RLS偏头痛组(78 例),分别为 90、46、78 例,比较三组血清β-EP、COX-2、DRD2 水平及SAS评分.采用Spearman相关系数分析RLS偏头痛组患者血清β-EP、COX-2、DRD2 水平与RLS分级、SAS评分的关系.结果 RLS偏头痛组血清β-EP、DRD2 水平均低于对照组,COX-2水平及SAS评分高于对照组,差异有统计学意义(P<0.05).中、大量RLS偏头痛组血清β-EP、DRD2 水平低于少量RLS偏头痛组,大量RLS偏头痛组低于中量RLS偏头痛组,差异有统计学意义(P<0.05).中、大量RLS偏头痛组血清COX-2 水平、SAS评分高于少量RLS偏头痛组,大量RLS偏头痛组高于中量RLS偏头痛组,差异有统计学意义(P<0.05).相关性分析结果显示,RLS偏头痛组患者血清β-EP、DRD2 水平与RLS分级、SAS评分呈负相关(rs<0,P<0.05),血清COX-2 水平与RLS分级、SAS评分呈正相关(rs>0,P<0.05).结论 随着RLS合并有先兆偏头痛患者RLS分级的升高,其血清β-EP、DRD2 表达降低,COX-2表达升高,且三者均与患者的焦虑状态密切相关.
Expression of β endorphin,cyclooxygenase-2,and dopamine receptor D 2 in serum of patients with right to left shunt and migraine with aura,and their association with anxiety
Objective To study the expression of β-endorphin(β-EP),cyclooxygenase-2(COX-2),and dopamine receptor D2(DRD2)in serum of patients with right to left shunt(RLS)and migraine with aura,and their relationship with anxiety.Methods A total of 214 patients with migraine with aura who were found to have RLS in Jilin Province FAW General Hospital from July 2022 to July 2023 were selected and recorded as the RLS migraine group,and 50 patients with migraine with aura without RLS were selected as the control group.Serum levels of β-EP,COX-2,DRD2,and self-rating anxiety scale(SAS)score were compared between the two groups.According to the examination results,the patients of RLS migraine group were divided into few,medium,and large RLS migraine group,with 90,46,and 78 cases,respectively.The serum β-EP,COX-2,DRD2 levels,and SAS scores of the three groups were compared.Spearman correlation coefficient was used to analyze the relationship between serum β-EP,COX-2,DRD2 levels,RLS grade,and SAS score in RLS migraine group.Results Serum β-EP and DRD2 levels in RLS migraine group were lower than those in control group,and COX-2 level and SAS score were higher than those in control group,the differences were statistically significant(P<0.05).Serum levels ofβ-EP and DRD2 in medium and large RLS migraine group were lower than those in few RLS migraine group,and those in large RLS migraine group were lower than those in medium RLS migraine group,the differences were statistically significant(P<0.05).Serum COX-2 level and SAS score in medium and large RLS migraine group were higher than those in few RLS migraine group,and those in large RLS migraine group were higher than those in medium RLS migraine group,the differences were statistically significant(P<0.05).Correlation analysis results showed that serum β-EP and DRD2 levels were negatively correlated with RLS grade and SAS score(rs<0,P<0.05),and serum COX-2 level was positively correlated with RLS grade and SAS score(rs>0,P<0.05).Conclusion With the increase of RLS grade in patients with RLS and migraine with aura,the expression of β-EP and DRD2in serum decreased,and the expression of COX-2 increased,and the three are closely related to the anxiety state of patients.
