目的 探讨侧方入路经皮椎间孔镜髓核摘除术(PTED)治疗钙化型腰椎间盘突出症(CLDH)的临床效果.方法 选择 2019 年 1 月至 2021 年 12 月安徽省马鞍山市人民医院脊柱骨科收治的 48 例CLDH患者为研究对象,采用随机数字表法将其分为观察组和对照组,各24例.观察组采用PTED技术治疗,对照组采用椎板开窗髓核摘除术治疗.比较两组手术时间、切口长度、住院时长.分别于术前及术后 1d、3个月和 1年时视觉模拟评分法(VAS)评分及Oswestry功能障碍指数(ODI)评分.结果 观察组手术时间、切口长度、住院时长短于对照组(P＜0.05).两组各时间点VAS评分、ODI评分比较,差异有统计学意义(P＜0.05).观察组VAS评分、ODI评分均低于对照组同期(P＜0.05).结论 PTED治疗CLDH的效果较好且PTED创伤更小、恢复更快,值得临床推广应用.
Clinical effect of percutaneous transforaminal endoscopic discectomy in the treatment of calcified lumbar disc herniation
Objective To investigate the clinical effect of percutaneous transforaminal endoscopic discectomy(PTED)in the treatment of calcified lumbar disc herniation(CLDH).Methods A total of 48 CLDH patients admitted to the Department of Spinal Orthopaedics,Maanshan People's Hospital,Anhui Province from January 2019 to December 2021 were selected as the study objects,and they were divided into observation group and control group by random number table method,with 24 cases in each group.Observation group was treated with PTED technique and control group was treated with posterior vertebral lamina fenestration discectomy.Operation time,incision length and hospital stay were compared between the two groups.Visual analogue Scale(VAS)score and Oswestry disability index(ODI)score were obtained before surgery and at one day,three months and one year after surgery,respectively.Results The operation time,incision length,and hospital stay in observation group were shorter than those in control group(P＜0.05).VAS score and ODI score were compared between the two groups at each time point,and the differences were statistically significant(P＜0.05).VAS score and ODI score in the observation group were lower than those in the control group(P＜0.05).Conclusion The efficacy of PTED in treating CLDH can further alleviate the symptoms than FD,and PTED is less traumatic and faster recovery,which is worthy of clinical application.
