Predictive value of transperineal three-dimensional ultrasound for pubic symphysis diastasis in primiparous women after childbirth
Objective To explore the predictive value of transperineal three-dimensional ultrasound for pubic symphysis diastasis(PSD)in primiparous women after childbirth.Methods A total of 105 patients diagnosed with postpartum PSD who delivered in the First Affiliated Hospital of Shihezi University from January 2022 to July 2023 were selected as the study group,and 105 healthy single term primiparous women who delivered during the same period were selected as the control group.Both groups underwent transperineal three-dimensional ultrasound examination of the pubic symphysis in the late pregnancy and postpartum period,the ultrasound indicators such as separation distance and left and right misalignment were compared between the two groups,and the diagnostic efficacy of ultrasound indicators for PSD was analyzed.Results Compared with the control group,the study group showed an increase in the widest and narrowest of the PSD,as well as the degree of left and right malocclusion,and 42 d postpartum visual analog scale score increased(P<0.05).The area under the curve of receiver operator characteristic for detecting PSD at the widest and narrowest separation of the pubic symphysis,as well as the combined detection of left and right malocclusion was larger than that of each indicator detected separately(P<0.05).Conclusion Transperineal three-dimensional ultrasound during pregnancy has high predictive value for postpartum PSD in primiparous women,and the pubic symphysis distance combined with left and right misalignment can accurately predict the occurrence of postpartum PSD.
