目的 探究类风湿关节炎(RA)患者血小板/血红蛋白比值(PHR)、白蛋白/纤维蛋白原比值(AFR)与疾病活动度的相关性分析.方法 选取2020年1月至2023年5月安徽省马鞍山十七冶医院收治的135例RA患者作为研究对象,根据疾病活动度将患者分为中/高度活动组95例及低度活动组40例,检测患者入院24 h的血小板、血红蛋白、白蛋白及纤维蛋白原水平,计算PHR、AFR.分析RA患者疾病活动度的影响因素,评估PHR、AFR对RA患者疾病活动度的预测价值.结果 中、高度活动组PHR水平高于低度活动组,AFR低于低度活动组(P<0.05).PHR(OR=5.652,95%CI:1.932~16.536)、AFR(OR=0.205,95%CI:0.069~0.598)是 RA 患者中/高度疾病活动度的独立影响因素(P<0.05).PHR、AFR单一及联合预测RA患者中/高度疾病活动度的受试者操作特征曲线下面积分别为0.728、0.716、0.809,且联合效能高于二者单独(P<0.05).结论 不同疾病活动度RA患者的PHR、AFR表达存在差异,PHR、AFR联合预测RA中/高度疾病活动度的预测效能良好.
Correlation analysis of platelet/hemoglobin ratio,albumin/fibrinogen ratio,and disease activity in patients with rheumatoid arthritis
Objective To explore the correlation between platelet/hemoglobin ratio(PHR),albumin/fibrinogen ratio(AFR),and disease activity in patients with rheumatoid arthritis(RA).Methods A total of 135 patients with RA admitted to the Maanshan Seventeen Metallurgical Hospital,Anhui Province from January 2020 to May 2023 were selected as the study objects.According to disease activity,the patients were divided into moderate/high activity group(95 cases)and low activity group(40 cases).The levels of platelet,hemoglobin,albumin,and fibrinogen were detected 24 h after admission,and PHR and AFR were calculated.The influencing factors of disease activity in patients with RA were analyzed,and the predictive value of PHR and AFR for disease activity in patients with RA was evaluated.Results PHR level in moderate/high activity group was higher than that in low activity group,and AFR level was lower than that in low activity group(P<0.05).PHR(OR=5.652,95%CI:1.932-16.536),and AFR(OR=0.205.95%CI:0.069-0.598)were independent influencing factors for moderate/high disease activity in RA patients(P<0.05).The area under the receiver operator characteristic curve of patients with moderate/high disease activity was 0.728,0.716,and 0.809 in PHR and AFR alone and combined,respectively,and the combined efficacy was higher than that of the two alone(P<0.05).Conclusion There are differences in the expression of PHR and AFR in RA patients with different disease activity,and the combination of PHR and AFR is effective in predicting moderate/high disease activity in RA.
