Analysis on the inpatient expenses and influencing factors of local patients in Tianjin hospital based on diagnosis-related group payment
Objective To analyze the inpatient expenses and influencing factors of local patients in Tianjin Hospital based on diagnosis-related group(DRG)payment.Methods Local inpatients cost settlement information and basic medical record information were selected from Tianjin hospital from January to December 2021 before DRG was implemented(control group),and from January to December 2022 after DRG was implemented(reform group).The general data,per capita total hospitalization cost,per capita daily hospitalization cost,and relative weight(RW)between two groups were compared;the influencing factors of per capita daily hospitalization cost were analyzed.Results A total of 24 140 cases were included in control group and 19 181 cases were included in reform group.There were statistically significant differences in gender and age between two groups(P＜0.05);there was no significant difference in hospital stay between two groups(P＞0.05).There were statistically significant differences in per capita total hospitalization cost,per capita daily hospitalization cost,and RW between two groups(P＜0.05);there were statistically significant differences in per capita total hospitalization cost,per capita daily hospitalization cost of material cost,drug cost,operation cost between two groups(P＜0.05).There were statistical significance in the per capita daily hospitalization cost of different DRG classification,gender,age,and RW(P＜0.05).DRG,DRG classification,gender,age,and RW were the influencing factors of per capita daily hospitalization cost(P＜0.05);and the multiple regression equation was:per capita daily hospitalization cost=10 408-428.097 × DRG reform group-242.224 × DRG material group+1 239.124× DRGdruggroup-576.581 × male+1 283.163 × age(60-＜80 years)+1 732.570 × age(80-＜90 years)+844.677 × age(≥90 years)-5 665.426 × RW(＜0.5)-5 154.877 × RW(0.5-＜0.8)-1 999.976 × RW(0.8-＜1.2).Conclusion After the imple ment-ation of DRG payment,the hospitalization cost of hospitalized patients have decreased,and the main factors affecting the per capita daily hospitalization cost are DRG,DRG classification,gender,age,and RW.
