燕京儿科学派论治抽动障碍的学术思想与经验传承探赜

摘要
燕京儿科学派立足于脏腑辨证,认为风痰为抽动障碍的致病关键,肝风内动为基本病机.学派泰斗刘弼臣教授及其传承人王素梅教授以"平肝息风"为治法基石,着眼于不同致病环节,分别形成了"从肺论治""从脾论治"两大代表性治法理念,对临床诊疗具有重要指导作用.刘弼臣教授聚焦于"外风引触肝风"始动环节,倡导调肺以截病于初,散外风以防内风亢动;王素梅教授承袭并发展其证治思路,聚焦于"脾虚"关键病理环节,倡导调脾以治病于本,健脾以平肝益肺,完善了本病反复发作期及恢复期的治法体系.本文总结燕京儿科学派两大代表性学术思想及观点演变中的传承发展,旨在进一步弘扬流派名医经验,并为本病证治提供思路和借鉴.
Abstract
Yenching Pediatrics School,based on syndrome differentiation of zang fu viscera,considered that the critical pathogenic factors of tic disorders were wind and phlegm,and the basic pathogenesis was liver wind stirring up internally.Professor Liu Bichen and his successor Professor Wang Sumei took"suppressing hyperactive liver for calming endogenous wind"as the foundation of treatment,they formed two representative treat-ment concepts of"treatment from lung"and"treatment from spleen"by focusing on different pathogenic links,which had important guiding effect on clinical diagnosis and treatment.Professor Liu Bichen focused on the starting point of"external wind triggering internal wind",advocated regu-lating the lung to intercept the disease at the beginning,and dispersing the external wind to prevent triggering the internal wind;Professor Wang Sumei inherited and further developed Professor Liu's academic thought,focused on the key pathology of"spleen deficiency",advocated regulating the spleen to treat the root,fortified the spleen to pacify the liver and tonify the lung,and improved the treatment system of the disease in the period of recurrent and recovery.This paper summarizes the two representative academic thoughts of Yencheing Pediatrics School and the inheritance and development in the evolution of views,aiming to promote the experience of famous doctors of the school and provide ideas and reference for the treatment of this disease.
