Construction and application of hierarchical management model of medi-cal and preventive integration in community hypertension patients
Objective To explore the construction and application of the hierarchical management model of medical and preventive integration un-der the compact medical community in the community hypertension patients.Methods A prospective randomized controlled study was conducted to select 243 patients with hypertension from West China Nursing College of Sichuan University and Caotang Community Health Service Center of Qingyang District of Chengdu from January 2021 to January 2022 as the study objects,and they were divided into combination group(122 cases)and control group(121 cases)by random number table method.The control group was managed by hierarchical management mode and the combi-nation group was managed by hierarchical management model of medical and preventive integration.The two groups were managed continuously for six months.Systolic and diastolic blood pressure were compared before and after three and six months of management.The health-promoting lifestyle profile-Ⅱ reformulate(HPLP-Ⅱ R)and Morisky questionnaire were used to evaluate the health behaviors and medication compliance of patients before and after six months of management.Results In this study,the control group and the combination group each shed one case.After three and six months of management,the systolic and diastolic pressure in both groups were lower than those before management,and the systolic and diastolic pressure in six months of management were lower than those in three months of management,and those of combined group were lower than those in the same period of control group(P<0.05).After six months of management,the HPLP-Ⅱ R scores of the two groups were higher than those before management,and those of combined group were higher than those of control group(P<0.05).After six months of management,the medication compliance of the two groups was better than that before management,and that of combination group was better than that of control group(P<0.05).Conclusion The hierarchical management mode of medical and preventive integration has a good application effect in the community patients with hypertension,which can effectively control patients'blood pressure,improve medication compliance,and enhance the level of health behavior.
