Influencing factors of the sense of self-burden and intervention counter-measures in radiotherapy patients with esophageal cancer
Objective To explore the influencing factors of the sense of self-burden and the intervention countermeasures in radiotherapy patients with esophageal cancer.Methods Convenience sampling method was used to select 90 esophageal cancer radiotherapy patients admitted to the Third Department of Radiotherapy,Huai'an First People's Hospital,Jiangsu Province,from January 2019 to October 2023 as study subjects.Basic information and caregiver-related information were collected from all patients,who were divided into two groups based on their self-perceived bur-den scale(SPBS)scores;SPBS scores<17 points were included in the no-low burden group(30 cases),and SPBS scores ≥ 17 points were includ-ed in the medium-high burden group(60 cases).Univariate analyses and binary logistic regression analyses were performed to construct a joint pre-diction model of independent risk factors and analyse its predictive performance.Results The results of univariate analysis showed that age,educa-tional level,monthly income,caregiver age,caregiver quality of life,caregiver psychological burden,caregiver underlying diseases in the medi-um-high burden group were significantly different from those in the no-low burden group(P<0.05).The results of binary logistic regression analy-sis showed that age(OR=1.135),education level of primary school or illiteracy(OR=7.276),caregiver age(OR=1.107),and caregiver psycholog-ical burden(OR=1.099)were all independent risk factors for developing a moderate to high level of burden in patients undergoing radiotherapy for oesophageal cancer(P<0.05),while caregiver quality of life(OR=0.980)was a protective factor(P<0.05).A predictive model was constructed based on the variables with P<0.05 in the multifactor analysis,and the results of the analysis of the subjects'operating characteristic curves showed that the area under the model curve=0.888(95%CI=0.817 to 0.959),suggesting that the model differentiation was good.The Hos-mer-Lemeshow test x2=8.947(P=0.347)suggested a good model fit;the results of the decision curve analysis showed that patients may have a higher positive net benefit by utilising the model when the threshold probability was in the range of 0.4 to 1.0.Conclusion Patients undergoing ra-diotherapy for esophageal cancer have a high level of self-burden,which is related to their age and education level,and is also affected by caregiver factors.For patients who may have a sense of self-burden,psychological support for caregivers should be carried out based on psychological coun-selling to reduce the level of self-burden of the patients.
