摘要
缺血性心肌病是一种严重的心血管系统疾病,发展到后期易导致心力衰竭的发生.中医药基于整体观念及辨证论治两大特点对于治疗此病具有相对优势.牛天福教授立足中医理论体系,从气血络脉心体系出发,分期辨治本病,认为本病始于气,延于血,长于络脉,终于心体,多相兼发病.提出本病气分病者调理气机,血分病者畅通血运,络脉病者涤顺络脉,心体病者顾护心体,相兼病者厘定轻重的治病大纲.治疗过程中,气分病又有气虚、气陷、气滞的不同.血分病分血虚、血瘀、痰瘀互结三阶段.络脉病有络脉空虚、络脉瘀堵两个方面.心体病注意顾护正气,相兼为病者根据其相兼病程的不同,选用不同方剂治疗.本文为缺血性心肌病心力衰竭的防治提供了新思路.另附医案一则供阅.
Abstract
Ischemic cardiomyopathy is a serious cardiovascular disease,which may lead to heart failure in the late stage of development.Traditional Chinese medicine has a relative advantage in treating this disease based on the two characteristics of holistic concept and dialectical treatment.Professor Niu Tianfu based on the theoretical system of Chinese medicine,starting from the system of qi,blood,collaterals,and heart,phased diagnosis and treatment of this disease,that this disease begins with qi,extends in blood,grows in collateral vessels,and ends in the heart,and multi-phase and concurrent disease.It is proposed that those who suffer from qi component of the disease should regulate the qi mechanism,those who suffer from blood component of the disease should unimpeded the blood transportation,those who suffer from collateral vessels should cleanse and smooth the collateral vessels,those who suffer from the body of the heart should take care of the heart body,and those who suffer from both phases of the disease should decide on the severity of the treatment outline.In the course of treatment,qi diseases have different qi deficiency,qi trapping,and qi stagnation.Blood diseases are divided into three stages:blood deficiency,blood stasis,and phlegm and stagnation.The diseases of the channels have two aspects:emptiness of the channels and stagnation of the channels.For heart disease,attention should be paid to the protection of positive qi,and different prescriptions should be used for the treatment of concomitant diseases according to the different stages of concomitant diseases.This article provides new ideas for the prevention and treatment of heart failure in ischemic cardiomyopathy.Another medical case is attached for reading.
