基于目标导向理论的达芬奇机器人手术标准化护理配合与管理体系的构建与应用

目的 探讨基于目标导向理论的达芬奇机器人手术标准化护理配合与管理体系的构建及应用效果。方法 采用文献回顾法、德尔菲法构建达芬奇机器人手术标准化护理配合与管理体系。采用非随机化的类实验研究设计,通过便利抽样选取2023年1月至3月于贵州医科大学附属医院手术室接受达芬奇机器人辅助手术的146例患者纳入对照组,2023年4月至6月的168例患者纳入试验组。对照组采用传统手术护理配合及管理方法,试验组采用手术标准化护理配合及管理方法。比较两组术前准备时间、手术持续时间、首台开台准点率、接台手术等待时间、手术配合度及医师满意度。结果 试验组术前准备时间、手术持续时间和接台手术等待时间均短于对照组,差异有统计学意义(P<0。05)。试验组首台开台准点率高于对照组,差异有统计学意义(P<0。05)。试验组手术间布局合理、仪器设备使用方便、手术体位对操作的影响、物品准备充分、器械摆台合理、器械传递准确、器械传递及时和医护配合评分均高于对照组,差异有统计学意义(P<0。05)。试验组手术物品准备、沟通、患者护理、故障处理、操作熟练度评分和总分均高于对照组,差异有统计学意义(P<0。05)。结论 手术标准化护理配合与管理可有效缩短达芬奇机器人手术术前准备时间、手术持续时间和接台手术等待时间,提高手术室护士手术配合度和医师满意度。
Construction and application of standardized nursing coordination and management system for DaVinci robotic surgery based on goal-oriented theory
Objective To explore construction and application effect of standardized nursing coordination and management system for DaVinci robotic surgery based on goal-oriented theory.Methods Standardized nursing coordination and manag-ement system for DaVinci robotic surgery were constructed by using literature review and Delphi method.Non randomized quasi experimental design was adopted.One hundred and forty-six patients underwent DaVinci robot assisted surgery in Operating Theater of Affiliated Hospital of Guizhou Medical University from January to March 2023 were selected as the control group through convenience sampling,and 168 patients from April to June 2023 were selected as the experimental group.The control group received traditional surgical nursing cooperation and management methods,while the experimental group received standardized surgical nursing cooperation and management methods.Preoperative preparation time,surgical duration,on time rate of first machine opening,waiting time for receiving surgery,surgical cooperation and doctor satisfaction between the two groups were compared.Results Preoperative preparation time,surgical duration and waiting time for receiving surgery in the experimental group were shorter than those in the control group,and the differences were statistically significant(P<0.05).On time rate of first machine opening in the experimental group was higher than that in the control group,and the difference was statistically significant(P<0.05).The scores of reasonable layout of operating room,convenient use of instruments and equipment,impact of surgical position on operation,sufficient preparation of items,reasonable placement of instruments,accurate transmission of instruments,timely transmission of instruments and medical and nursing cooperation in the experimental group were higher than those in the control group,and the differences were statistically significant(P<0.05).The scores of surgical item preparation,communication,patient care,fault handling and operational proficiency and total scores in the experimental group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Standardized nursing coordination and management in surgery can effectively shorten preoperative preparation time,operation duration and waiting time for subsequent surgeries in DaVinci robotic surgery,and improve cooperation degree of operating theater nurses and doctor satisfaction.
