Exploration of chronic disease whole-process management mode of"doctor,nurse and health manager"integration in general public hospitals
Chronic diseases are the main causes of death in Chinese.The deepening of aging and the trend of younger patients have intensified the situation of chronic disease prevention and control.The current prevention and control mode of chronic diseases based on primary medical institutions is not ideal,which can't meet the needs of patients with chronic diseases for high-quality medical resources.To help solve the medical problem of chronic disease patients and to form the prevention and fusion medical service mode of public hospital,West China Hospital of Sichuan University explore constructed"doctor,nurse and health manager"integration team collaborative chronic disease whole-process management mode.And this mode provides a new solution to strengthen chronic disease management.
