Study on the current status and influencing factors of depression in first-treatment pulmonary tuberculosis patients based on random forest model-ing
Objective To investigate the current status of depression in first-treatment pulmonary tuberculosis patients and explore its influencing factors,so as to provide a basis for clinical psychological intervention.Methods A convenience sampling method was used to select 208 pulmonary tuberculosis patients who were hospitalized in the Department of Respiratory Medicine of the Fourth People's Hospital of Ningxia Hui Autonomous Region from December 2023 to February 2024,and met the inclusion and exclusion criteria.General information questionnaire,hospital anxiety and depression scale,chronic disease self-management ability measurement scale,and perceived social support scale were used to investigate.Random forest model and logistic regression were applied to analyze the influencing factors of patients'depression.Results The depression score of pulmonary tuberculosis patients was(7.44±3.81)points,the score of self-management ability was(163.24±25.42)points,and the score of perceived social support was(61.21±11.54)points;depression was negatively correlated with self-management ability and perceived social support,respectively(r=-0.333,-0.357,P<0.01);the detection rate of depression was 50.96%(106/208);the results of univariate analysis showed that the differences in the number of cases of depression detection among pulmonany tuberculosis patients with different ages,education levels,per capita monthly family income,marital status,number of symptom types,and number of other chronic disease types were statistically significant(P<0.05);and the random forest model importance rankings in descending order were self-management ability,perceived social support,per capita monthly household income,age,education levels,and number of symptom types.The results of logistic regression analysis showed that self-management ability,perceived social support,age,and the number of symptom types were the influencing factors of depression in patients with tuberculosis(P<0.05).Conclusion The detection rate of depression in first-treatment pulmonary tuberculosis patients is high,and should focus on patients with lower self-management ability,poor perceived social support,the elderly,and patients with more clinical symptoms,and provide them with psychological care and support to reduce their depression.
