Effect of 3D proprioceptive insole on lower limb motor ability and muscle status in patients with foot drop and varus after stroke
Objective To investigate the effect of 3D proprioceptive insole on the lower limb on lower limb motor ability and muscle status in patients with foot drop and varus after stroke.Methods A total of 60 patients with foot drop and varus after stroke treated in the Department of Neurological Rehabilitation,the Fourth Rehabilitation Hospital of Shanghai from December 2021 to April 2024 were selected.They were randomly divided into 3D proprioceptive insole group(insole group)and ankle-foot orthosis group(AFO group)by computer randomly method,with 30 cases in each group.On the basis of conventional treatment,the insole group received 3D proprioceptive insole treatment and the AFO group received AFO treatment.Both groups were treated contuinuously for six weeks.the Berg balance scale(BBS),Fugl-Meyer assessment(FMA),timed up and Go test(TUGT),gait analysis,and surfaces electromyographic were evaluated in the two groups before and after treatment.Results After treatment,the BBS score and FMA score of both groups were higher compared with before treatment,the TUGT was lower compared with before treatment,the BBS score and FMA score of the insole group were higher than the AFO group,the TUGT was lower than the AFO group(P<0.05).After treatment,the gait speed of the two groups was higher compared with before treatment,the gait cycle was shorter compared with before treatment,the gait speed of the insole group was higher than the AFO group,the gait cycle was shorter than the AFO group(P<0.05).The stance phase of affected side from the two groups were decreased compared with before treatment,increased in swing phase compared with before treatment,and the stance phase of insoles group was lower than the AFO group,the swing phase of insoles group was higher than the AFO group(P<0.05).After treatment,the tibialis anterior,lateral gastrocnemius integrated electromyography(iEMG)of affected side of the two groups were improved compared with before treatment,the co-contraction rate(CCR)was down compared with before treatment,and the tibialis anterior,lateral gastrocnemius iEMG of insoles group were higher than the AFO group,the CCR of insole group was lower than the AFO group(P<0.05).Conclusion Compared with AFO,3D proprioceptive insole is more effective in promoting muscle co-contraction,improving balance function,correcting gait,and restoring motor ability.
