HIF-1α介导针灸调节氧化应激修复肠易激综合征肠黏膜屏障策略与机制探讨

摘要
肠易激综合征(IBS)发病率高,临床多以止泻、解痉止痛、抗炎和调节肠道菌群等对症治疗为主.氧化应激与IBS病理密切相关,可导致线粒体功能障碍、DNA修复受损及细胞损伤或凋亡.氧化应激状态下低氧诱导因子1α(HIF-1α)介导肠上皮-巨噬细胞对话参与IBS生理病理过程,是肠黏膜屏障损伤潜在治疗靶点,阻断HIF-1α显示极富前景的治疗潜力.本文结合近10年研究进展,从HIF-1α介导肠上皮-巨噬细胞对话,参与IBS肠黏膜屏障损伤形成角度探讨针灸保护IBS肠黏膜屏障的可能机制,为深入揭示针刺治疗IBS效应机制提供新的科学依据,进一步丰富针灸学的理论内涵.
Abstract
Irritable bowel syndrome(IBS)has a high incidence rate,and clinical treatment is mainly symptomatic treatment such as antidiarrheal,antispasmodic and analgesic,anti-inflammatory,and regulation of intestinal flora.Oxidative stress is closely related to the pathology of IBS and can lead to mitochondrial dysfunction,impaired DNA repair,cell damage or apoptosis.Hypoxia-inducible factor 1α(HIF-1α)mediated intestinal epithelial-macrophage dialogue under oxidative stress participates in physiological and pathological process of IBS.It is a potential therapeutic target for intestinal mucosal barrier injury,blocking HIF-1α shows promising therapeutic potential.Based on the past 10 years research progress,this paper discusses the possible mechanism of acupuncture in repairing intestinal mucosa barrier from the perspective of HIF-1 αmediated intestinal epithelial-macrophage dialogue,which provides a new scientific basis for further revealing the mechanism of the effect of acupuncture on IBS,and enriches theoretical connotation of acupuncture.
