资质准入对公立医院互联网诊疗业务的影响分析

目的 分析医院取得互联网诊疗资质前后线上业务变化,为科学管理提供参考.方法 以获批资质为节点,将2018 年 2 月至 2022 年 11 月分为三个阶段,结合管理举措,以间断时间序列法分析业务变化.以回归系数t检验分析线上工作量变化.以卡方检验比较线上业务完成率.结果 取得互联网诊疗资质后,实际发起需求的患者占比由 30.14%提升到 41.75%(p<0.001).取得互联网医院资质后,实际发起需求的患者占比下降到 32.97%(p<0.001);咨询需求量由逐月增长 131 人次变为下降 62 人次(p<0.05);复诊需求及完成量每月分别上升 108 及 102 人次(p<0.001).该阶段采取了严格医师资质管理及调整绩效方案等措施.结论 互联网诊疗资质准入为线上业务的发展创造了政策条件,管理举措可将政策机遇转化为业务实质发展.
Abstract
Objective To analyze the changes of online medical services before and after the hospital obtains the qualification of online diagnosis and treatment,and to provide reference for scientific management.Methods The business changes were analyzed by interrupted time series method by dividing the period from February 2018 to November 2022 into three phases combined with management initiatives using the approved qualification as a node.Analyzed online workload changes with regression coefficient t-test.Compared online business completion rates with chi-square test.Results After obtaining the qualification of online diagnosis and treatment,the proportion of patients who actually initiated the demand increased from 30.14%to 41.75%(p<0.001).After obtaining the qualification of Internet hospital,the proportion of patients who actually initiated demand decreased to 32.97%(p<0.001).The demand for consultation changed from a monthly increase of 131 to a decrease of 62(p<0.05).The demand for and completion of follow-up consultations increased by 108 and 102 per month,respectively(p<0.001).Measures such as strict physician qualification management and adjusting the performance program were taken at this stage.Conclusion Internet diagnosis and treatment qualification approval creates policy conditions for the development of online service,and management initiatives can turn policy opportunities into substantial business development.
