温州地区RhD阳性患者实行Rh表型同型/相容型输血的可行性研究

摘要
目的 探讨温州地区RhD阳性患者实行Rh表型同型/相容性红细胞输血的可行性.方法 采用全自动微柱凝胶法检测 279910 名Rh(D)阳性患者的Rh表型,获得C/c/E/e抗原阳性率和Rh表型构成比,通过SPSS软件进行统计分析.结果 279910名Rh(D)阳性患者中共检测到9种Rh表型,不同Rh表型实现同型/相容型输血的概率分别为CcEe(100%)、Cce(55.04%)、CEe(46.67%)、Ce(45.95%)、cEe(10.11%)、CcE(6.62%)、cE(6.47%)、ce(0.30%)、CE(<0.01%).当红细胞库存≥46 袋时,除ce、CE两种稀有表型外,其余 7 种表型均可实现同型/相容型输注.结论 温州地区RhD阳性患者开展Rh表型同型/相容性红细胞输血具有可行性,为了进一步提高临床同型/相容型输血率,实现精准输血,推进安全用血,建议血站建立Rh表型库并根据输血科需求发放相应Rh表型血液.
Abstract
Objective To investigate the feasibility of Rh homotypic/compatible red blood cell transfusion in RhD-positive patients in Wenzhou.Methods The Rh phenotypes of 279910 Rh(D)-positive patients were detected by automatic microcolumn gel method,and the positive rate of C/c/E/e antigens and the composition ratio of Rh phenotypes were obtained.SPSS software was used for statistical analysis.Results A total of 9 Rh phenotypes were detected in 279910 Rh(D)-positive patients.The probabilities of achieving homotypic/compatible transfusion for different Rh phenotypes were CcEe(100%),Cce(55.04%),CEe(46.67%),Ce(45.95%)、cEe(10.11%),CcE(6.62%),cE(6.47%),ce(0.30%)and CE(<0.01%).When the red blood cell inventory was more than 46 bags,except for the rare phenotypes of ce and CE,the other 7 phenotypes could be used for homotypic/compatible transfusion.Conclusion It is feasible to perform Rh phenotype homotypic/compatible red blood cell transfusion for RhD positive patients in Wenzhou.In order to further improve the clinical homotypic/compatible transfusion rate,achieve accurate transfusion,and promote safe blood use,it is recommended that blood banks establish Rh phenotype banks and distribute Rh phenotype blood according to the needs of the transfusion department/blood bank.
