Application of Targeted Next-Generation Sequencing Using Bronchoalveolar Lavage Fluid Combined with Galactomannan Detection in the Diagnosis of Non-Neutropenic Invasive Pulmonary Aspergillosis
Objective To evaluate the efficacy of targeted next-generation sequencing ( tNGS) using bronchoalveolar lavage fluid ( BALF) combined with galactomannan ( GM) detection in the diagnosis of non-neutropenic invasive pulmonary as-pergillosis ( IPA) .Methods A total of 60 suspected non-neutropenic IPA patients admitted to the Taihe Hospital,Hubei University of Medicine between April 2022 and May 2024 were enrolled.All patients underwent serum GM testing,BALF GM testing,and BALF tNGS.The sensitivity,specificity,positive predictive value ( PPV),and negative predictive value ( NPV) of each diagnostic method were compared.Receiver operating characteristic ( ROC) curve analysis was performed to calculate the area under the curve ( AUC) of each method and assess the diagnostic performance.Results The sensitivity (94.7%) and specificity (95.5%) of the combined BALF tNGS and BALF GM testing were significantly higher than those of serum GM,BALF GM,or BALF tNGS alone.The AUC of the combined testing was 0.95,which was higher than that of the individual methods (P<0.05).Conclusion The combination of BALF tNGS and BALF GM testing provides superior performance compared with individual methods in diagnosing patients with non-neutropenic IPA,which significantly en-hances diagnostic accuracy and holds significant clinical value.
