The inter-relationship and weight of diagnostic elements of autoimmune encephalitis
Objective To analyze the weights of limbic system symptoms,epilepsy,inflammatory cerebrospinal fluid(CSF),brain MRI-FLAIR findings,and video electroencephalogram(VEEG)findings in diagnosing autoimmune en-cephalitis(AE).Methods We calculated the mean weight rates(%)of limbic system symptoms,epilepsy,inflamma-tory CSF,brain MRI-FLAIR findings,and VEEG findings of 15 cases of AE admitted to our hospital from January to July 2023.Results The frequencies of the elements were as following:limbic system symptoms,14/15 cases;epilepsy,7/15 cases;inflammatory CSF,6/13 cases(all the 6 positive cases had increased lymphocyte and monocyte counts and/or in-creased microprotein levels;5 cases had normal white blood cell counts,but with increases in lymphocytes and monocytes indicated by CSF cytology analysis;and 2 cases were not examined);MRI-FLAIR abnormalities,13/15 cases;and VEEG abnormalities,12/15 cases.All the patients with seizures had both limbic system symptoms and abnormal MRI-FLAIR sig-nals in the limbic system,and abnormal brain regions on MRI-FLAIR all showed abnormal discharges on VEEG.The mean weight rates of limbic system symptoms,brain MRI-FLAIR abnormalities,and VEEG abnormalities were the high-est,which were 94.2%,96.4%and 79.9%,respectively.Conclusion Limbic system symptoms,abnormal VEEG abnor-malities,and MRI-FLAIR abnormalities are most frequent in AE.Cytology analysis is necessary for inflammatory CSF ex-amination.Seizures with limbic system symptoms are highly indicative of limbic encephalitis.
