工作量增加背景下工作压力及职业倦怠对住院医师核心胜任力的影响

摘要
目的 了解临床工作压力及职业倦怠对住院医师六大核心胜任力的影响.方法 通过发放线上问卷,收集在培住院医师的一般信息、职业倦怠信息及临床工作量增加前后职业素养、专业能力、病人管理、沟通合作、教学能力、学习提升六大核心胜任力等信息,比较工作量增加背景下住院医师六大核心胜任力变化情况,分析其相关因素.结果 在经历 1 个月的临床工作量增加后,除职业素养外,住院医师其他五大核心胜任力均有显著的提升(均P<0.05);每周工作时长在 50～59 小时的住院医师在病人管理、沟通合作以及学习提升方面获得的提升大于每周工作时长≥60 小时的住院医师(P<0.05);职业倦怠程度与住院医师在沟通合作以及学习提升方面的进步呈负相关(P<0.05).结论 临床工作压力增加对住院医师胜任力提高有积极影响,适度的工作时长有助于胜任力的提升,但每周工作时长过长和职业倦怠程度过高可能会阻碍住院医师核心胜任力的提高.
Abstract
Objective To explore the impact of clinical work pressure and occupational burnout on the six core competencies of resident physicians.Methods Online questionnaires were issued to collect general information,occupational burnout level,and six core competencies on professionalism,professional ability,patient management,communication and cooperation,teaching skill,and learning to enhance capability of resident physicians before and after the increase of workload.Changes in the six core competencies of resident physicians were compared under the background of increased workload and their related factors were analyzed.Results After experiencing an increase in clinical workload for one month,the other five core competencies of resident physicians except for professionalism were significantly improved(P<0.05).Resident physicians who worked 50～59 hours per week showed greater improvement in patient management,communication and collaboration,and learning to enhance capability compared with those who worked 60 hours per week(P<0.05).The degree of occupational burnout was negatively correlated with the improvement of communication and cooperation and learning to enhance capability(P<0.05).Conclusion The increase in clinical work pressure has a positive impact on the competency enhancement of resident physicians.Moderate working hours can help enhance competence,but excessively long weekly working hours and high levels of occupational burnout may hinder the improvement of core competencies of resident physicians.
