基于Mini-CEX评估三轮分层递进培养模式下住院医师产科轮转效果

摘要
目的 探讨三轮分层递进培养模式在产科轮转住院医师教学中的效果.方法 纳入于复旦大学附属妇产科医院产科轮转的 2020 级和 2021 级 66 名住院医师,2021 级住院医师(n=36)接受三轮分层递进培养,2020 级住院医师(n=30)接受两轮阶段性培养,采用迷你临床演练评估(mini-clinical evaluation exercise,Mini-CEX)方法在每轮培养结束时评估住院医师的产科临床能力,比较 2020 级和 2021 级Mini-CEX测评结果,收集住院医师每次测评后反馈信息、考官和住院医师对考核的满意度.结果 2020 级住院医师在病史采集、人文关怀、整体表现方面较首次测评有提高(均P<0.05).2021 级住院医师在三轮测评中各方面均有提高(均P<0.05).其中通过多次阶段性评估和不断反馈,两组住院医师第一轮Mini-CEX测评差异无统计学意义(P>0.05),最后一轮Mini-CEX测评中 2021 级住院医师在病史采集、诊疗思路和整体表现方面得分更高,差异有统计学意义(均P<0.05).不同轮转程度的住院医师测评后反馈的侧重点有所不同,住院医师和考官对考核的满意度均达到 100%.结论 三轮分层递进培养模式在住院医师产科轮转中效果明显,值得在临床教学中应用和推广.
Abstract
Objective To explore the effect of three-rounds of hierarchical and progressive training in the obstetrics residents training during their rotation.Methods A total of 66 residents rotating through obstetrics at the Obstetrics and Gynecology Hospital of Fudan University,including Grade 2020(n=30)and Grade 2021(n=36).The Grade 2021 underwent three-rounds of hierarchical and progressive training,while the Grade 2020 received two-rounds of phased training.Mini-CEX method was used to evaluate the obstetric clinic competencies of residents at the end of each training round.The evaluation results of the two grades were observed and compared,and feedback from residents after assessment,as well as the satisfaction of examiners and resident physicians with the evaluation process,were collected.Results The level of medical history collection,humanistic care and overall performance of Grade 2020 residents were higher than those of the first evaluation(all P<0.05).The Grade 2021 residents were improved in all aspects of the three-round evaluation(all P<0.05).Through multiple phased assessments continuous feedback,there was no statistically significant difference between the two grades in the first round of Mini-CEX evaluations(P>0.05).In the final round of Mini-CEX evaluations,the Grade 2021 residents scored higher in the aspects of medical history collection,clinical thinking and overall performance(all P<0.05).The focus of feedback from residents after each evaluation varied depending on the level of rotation.The satisfaction of both the residents and the examiners with the evaluation reached 100%.Conclusion The three-round hierarchical progressive training mode demonstrates significant effectiveness in the obstetric rotation for residents and is worthy applying and promoting in clinical teaching.
