基于TBL的分层教学模式在神经内科住院医师规范化培训中的应用

摘要
目的 探讨基于团队的学习(team-based learning,TBL)的分层教学模式在神经内科住院医师规范化培训(简称住培)中的应用效果.方法 选取 2021 年 7 月—12 月及 2022 年 7 月—12 月在浙江省台州医院神经内科轮转的住院医师 119 人为研究对象,2021 年为观察组,采取分层递进TBL教学法;2022 年为对照组,采取传统教学法.比较两组在教学满意度及综合能力考核两方面的培训效果.结果 在教学满意度评价方面,指导医师、教学秘书对观察组的满意度评价高于对照组,差异有统计学意义(P<0.05),观察组对科室满意度评价高于对照组,差异有统计学意义(P<0.05).综合能力考核方面,观察组的SOAP、理论成绩、体格检查、临床思维和病史采集考核的成绩高于对照组,差异有统计学意义(P<0.05).结论 基于TBL的分层教学模式在神经内科住培中的应用可以满足不同层次住院医师的要求,提高总体满意度及考核成绩,增强住院医师主动学习及临床思维能力.
Abstract
Objective To explore the effect of the hierarchical teaching mode based on TBL(team-based learning)in the standardized training of neurology residents.Methods A total of 119 residents who rotated in the Neurology Department of Taizhou Hospital of Zhejiang Province from July to December 2021 and from July to December 2022 were selected as the study subjects.Residents in 2021 were enrolled in the hierarchical TBL teaching group(observation group),and residents in 2022 were enrolled in the traditional teaching group(control group).The training effects of the two groups in teaching satisfaction and comprehensive ability assessment were compared.Results In terms of teaching satisfaction evaluation,the guidance doctors and teaching secretaries had higher satisfaction in the observation group than in the control group(P<0.05).The satisfaction evaluation of department in the observation group was higher than that in the control group,the difference was statistically significant(P<0.05).In terms of comprehensive ability assessment,the scores of SOAP,theory,medical history collection,physical examination,clinical thinking and medical history collection in the observation group were significantly higher than those in the control group(P<0.05).Conclusion The application of TBL based hierarchical teaching model in the standardized training of neurology residents can meet the requirements of residents at different levels,improve the overall satisfaction and assessment results,and enhance the students'active learning and clinical thinking ability.
