基于OBE的持镜助手培训在腹腔镜前列腺癌根治术临床教学中的应用

摘要
目的 探讨基于成果导向教育(outcome based education,OBE)的持镜助手培训在腹腔镜前列腺癌根治术(laparoscopic radical prostatectomy,LRP)中的应用.方法 选取 2022 年 7 月—2022 年 12 月上海交通大学医学院附属瑞金医院外科专业基地轮转的第一年住院医师 60 人为研究对象,随机分为观察组(30人)和对照组(30 人).观察组采取OBE模式培训,对照组采取自主训练,培训前后进行考核评分.结果 培训前后比较,LRP持镜能力得分显著提高;两组比较,观察组的得分[10.0(0.0)]高于对照组[9.0(2.0)],差异有统计学意义(Z=-4.42,P<0.001),且观察组住院医师持镜能力更均衡.观察组住院医师LRP相关解剖和手术过程认识方面得分高于对照组,差异有统计学意义(均P<0.001).结论 基于OBE模式的LRP持镜助手系统培训,能显著提高外科专业基地住院医师的持镜能力和理论知识学习效果,值得推广.
Abstract
Objective To investigate the significance of OBE model system for training camera holder of laparoscopic radical prostatectomy(LRP).Methods The first-year residents of the Ruijin Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from July 2022 to December 2022 were randomly divided into the observation group(n=30)and control group(n=30).The observation group received OBE training and the control group received autonomous training,and the effects were determined.Results In both the observation group and the control group,the score of camera-holding training improved significantly from the baseline.The scores of observation group was higher than that of control group[10.0(0.0)versus 9.0(2.0),Z=-4.42,P<0.001],and the camera holding ability was more balanced among residents in observation group.The scores of LRP related anatomy and operation process in observation group were significantly higher than those in the control group(P<0.001).Conclusion The training system for LRP camera holding assistants based on OBE model can improve the residents'camera holding ability and the effect of learning theoretical knowledge significantly,and its promotion should be considered.
