摘要
原位模拟是一种基于真实的临床诊疗环境,运用情境化模拟案例对住院医师进行培训的方式.不仅能利用工作间隙提升住院医师的技术性技能和非技术性技能,还能评估临床环境及医疗流程,识别出医疗系统中的潜在威胁.然而,原位模拟在应用过程中也面临着诸多挑战,不但需要应对医疗环境中存在的众多不可控因素,甚至可能对真实患者的安全构成威胁.在设计阶段,根据学员特点制定教学目标、设计情境案例并选择适合的培训设备,在实施过程中,做好评价、反馈及复盘工作至关重要.本文概述了原位模拟的概念和它在住院医师临床技能培训中的应用优势与挑战,以及设计和实施要点.
Abstract
In-situ simulation,a training method for resident physicians based on real clinical settings,uses contextualized simulation cases to enhance both their technical and non-technical skills.It not only takes advantage of idle time to improve the skills of resident physicians but also assesses the clinical environment and medical processes,identifying potential threats in the medical system.However,in-situ simulation also faces numerous challenges during application,including dealing with many uncontrollable factors in the medical environment and even posing a threat to the safety of real patients.During the design stage,it is essential to set teaching goals based on the characteristics of the trainees,design contextualized cases,and select appropriate training equipment.In the implementation process,conducting evaluations,providing feedback,and conducting debriefings are crucial.This article summarizes the concept of in-situ simulation,its advantages and challenges in the clinical skills training of resident physicians,as well as key points in design and implementation.
