Dynamic observation of serum thyroid hormone level in patients with chronic obstructive pulmonary disease in different course and prognosis
Objective To investigate the significance of serum thyroid hormone level in patients with chronic obstructive pulmonary disease(COPD)in different course and prognosis.Methods From January 2020 to June 2022,our hospital diagnosed and treated 179 patients with acute exacerbation of COPD(the observation group).According to 1∶1,179 physical examiners in the physical examination department of our hospital who received routine physical examination were selected as the control group.Take 5 ml blood from fasting vein,and use radioimmunoassay to detect serum thyroxine(TT4),serum free thyroxine(FT4),serum triiodothyronine(TT3),serum free triiodothyronine(FT3),and serum thyroid stimulating hormone(TSH).Results TT3,FT3,TT4 and FT4 of patients in the observation group during tacute attack period were lower than the control group,and TSH was higher than the control group(P<0.05).Among 179 patients,11 patients died.The levels of TT3,FT3,TT4 and FT4 in 168 discharged patients with improvement were higher than 179 patients at admission during acute attack(P<0.05).TSH was lower than the level at admission during acute attack(P<0.05).The levels of TT3,FT3,TT4 and FT4 in 11 dead patients were lower than 168 survivors(P<0.05).TSH level was higher than that of the survival group(P<0.05).Conclusion The level of thyroid hormone in patients with chronic obstructive pulmonary disease is significantly different in different course and prognosis.The dynamic change of serum thyroxine can be used as an observation indicator for the occurrence,development,therapeutic effect and prognosis of chronic obstructive pulmonary disease.
