Rationality evaluation on the application of polymyxin E sulfate in severe infection patients based on attribute hierarchical model
Objective To establish the rationalityevaluation criteria for polymyxin E sulfate in severe infection patients,and comprehensively evaluate the rationality of clinical practice of polymyxin E sulfate based on attribute hierarchical model(AHM).Methods Based on the drug instruc-tions,related clinical guidelines or expert consensus,the rationality evaluation criteria of polymyxin E sulfate were established.The archived cases of 75 patients treated with polymyxin E sulfate for injection in Northern Jiangsu People's Hospital from January 2021 to December 2022 were analyzed and evaluated by AHM.Results The main unreasonable types of clinical application of polymyxin E sulfate were inappropriate choice of the solvent,long course of administration and unreasonable combination of medication.26 patients(35%)with MRS<80 points,6 patients(8%)with 80≤MRS<80 points,20 patients(27%)with 85≤MRS<90 points,and 23 patients(31%)with MRS≥90 points.The average score of MRS was 82.86.Conclusion The rational use of polymyxin E sulfate in this hospital is not up to the mark,and the management should be strengthened to improve its safety and effectiveness.The AHM with the characteristic of result quanyified provides a novel tool for drug rationality evaluation,keeping its evaluation systematic,feasible and scientific.
