Application and influencing factors analysis of expanded pharmaceutical follow-up based on chronic disease management in hypertensive patients
Objective On the basis of chronic disease management,provide expanded pharmaceutical follow-up intervention to hypertensive patients,evaluate the blood pressure control effect of hypertensive patients before and after the intervention,and explore its influencing factors.Methods 116 hypertensive patients who received treatment at a medical institution in the local area from March 2020 to June 2022 were selected to establish a pharmaceutical service record and receive expanded pharmaceutical follow-up intervention for a period of 6 months.A control study was conducted on each patient before and after their own intervention to observe the changes in blood pressure,lifestyle,and single and multiple factor analysis that affect blood pressure control before and after the investigation.Results On the basis of chronic disease management,expanding pharmaceutical follow-up intervention was given to hypertensive patients.The number of hypertensive patients who did not take medication signif-icantly decreased(P<0.05).In terms of medication compliance,the number of regular medication patients significantly increased(P<0.05),and the number of patients who missed or did not take medication significantly decreased(P<0.05);The number of hypertensive patients who master rational medication,comply with medical standards,and meet blood pressure control standards has significantly increased(P<0.05);The results of univariate analysis showed that age,course of disease,number of medications used after intervention,frequency of medication used after intervention,and medication adherence after intervention were the influencing factors on the satisfaction of blood pressure control in hypertensive patients(P<0.05);Multivariate logistic analysis showed that patient age≥65 years old,medication frequency after intervention at 1 or≥2 times per day,and adherence to medication after intervention were independent influencing factors for blood pressure control in patients(P<0.05).Conclusion Providing expanded pharmaceutical follow-up intervention to hypertensive patients on the basis of chronic disease management can improve their medication compliance and improve their medication patterns.On the other hand,it can significantly help patients improve their compliance with medical advice and better control blood pressure.Providing expanded pharmaceutical follow-up intervention to hypertensive patients on the basis of chronic disease management is of great significance.
