194例感染性眼内炎临床特点与病原学分析

摘要
目的 探讨感染性眼内炎的致病因素、病原菌分布、耐药性以及手术与药物治疗手段.方法 回顾性分析 2019 年 1 月～2021 年12 月就诊于河北省眼科医院感染性眼内炎 194 例患者的临床资料.结果 眼外伤是感染性眼内炎的首要致病因素,其次是眼部手术.革兰阳性菌为引起感染性眼内炎最主要的致病菌,其中表皮葡萄球菌最多,对万古霉素、阿米卡星、氯霉素、利福平敏感性良好.玻璃体内注药术与玻璃体切除术为主要治疗手段.结论 眼内炎为严重致盲性眼病,明确感染性眼内炎病原菌及耐药性有助于及时经验性用药,同时积极有效的玻璃体切割手术也有助于取得更好的治疗效果.
Abstract
Objective To investigate the pathogenic factors,pathogen distribution,drug resistance and surgical and drug treatment of infec-tious endophthalmitis.Methods The clinical data of 194 patients with infectious endophthalmitis treated in Hebei ophthalmology hospital between January 2019 and December 2021 were analyzed retrospectively.Results Eye trauma was the primary pathogenic factor of infectious endophthalmitis,followed by eye surgery.Gram-positive bacteria is the main pathogen causing infectious endophthalmitis,of which staphylococcus epidermis is the most common,with good sensitivity to vancomycin,amikacin,chloramphenicol and rifampicin.Intravitreous injection and vitrectomy are the main methods of treatment.Conclusion Endoophthalmitis is a serious blindness,and the identification of infectious endophthalmitis pathogen and drug resistance is helpful for the timely and empirical treatment,while active and effective vitrectomy is also helpful for better therapeutic effect.
