不同临床分型冠心病的中成药合理用药分析

目的 分析不同临床分型冠心病的中成药合理用药情况，规范患者合理、安全用药。方法 收集某院 2020 年 12 月～2023 年 12月采用中成药治疗的冠心病患者 330 例，并抽取所有患者中成药治疗的处方，分析这些处方的用药类型、相关药物销售金额及不合理处方。结果 本研究中所选取的冠心病患者采用中成药类型排行前三的为行气活血剂、益气活血剂、化瘀通脉剂，分别占比为 30%、25。15%、9。7%。本研究中冠心病中成药销售金额前十的药品分别为脑心通胶囊、稳心颗粒、速效救心丸、复方丹参滴丸、血塞通软胶囊、通心络胶囊、银杏叶片、参松养心胶囊、麝香保心丸、血脂康胶囊。分析显示，药物使用强度(DDDs)排行前三位的为复方丹参滴丸、速效救心丸、脑心通胶囊，而日均费用(DDC)排行前三位的为银杏叶片、血脂康胶囊、复方丹参滴丸;这些处方中不合理用药的原因分别为缺少中医辨证、重复用药、疗程不当、辨证不当，占比分别为 3。94%、2。42%、1。52%、0。91%。结论 某院用于治疗冠心病的中成药种类较多，且基本合理，但仍存在不合理的现象，需要药师进行干预，加强合理用药。
Analysis of rational drug use of Chinese patent medicines in different clinical types of coronary heart disease
Objective To analyze the rational drug use of Chinese patent medicines in different clinical types of coronary heart disease,and to standardize the rational and safe use of drugs by patients.Methods From December 2020 to December 2023,330 patients with coronary heart disease treated with Chinese patent medicines in a hospital were collected,and the prescriptions of all patients treated with Chinese patent medicines were extracted,and the types of drugs used in these prescriptions,the sales amount of related drugs,and unreasonable prescriptions were analyzed.Results The top three types of Chinese patent medicines used by patients with coronary heart disease in this study were Qi-activating and blood-activating agents,Qi-invigorating and blood-activating agents and Huayu Tongmai agents,accounting for 30%,25.15%and 9.7%respectively.In this study,the top ten drugs in the sales amount of Chinese patent medicines for coronary heart disease are Naoxintong Capsule,Wenxin Granule,Suxiao Jiuxin Pill,Compound Danshen Dripping Pill,Xuesaitong Soft Capsule,Tongxinluo Capsule,Ginkgo Leaf Pill,Shensongyangxin Capsule,Musk Baoxin Pill and Xuezhikang Capsule.The analysis shows that the top three DDDs are Compound Danshen Dripping Pill,Suxiao Jiuxin Pill and Naoxintong Capsule,while the top three DDC are Ginkgo Leaf Pill,Xuezhikang Capsule and Compound Danshen Dripping Pill.The reasons for irrational drug use in these prescriptions are lack of TCM dialectics,repeated drug use,improper course of treatment and improper dialectics,accounting for 3.94%,2.42%,1.52%and 0.91%respectively.Conclusion There are many kinds of Chinese patent medicines used to treat coronary heart disease in the hospital,which are basically reasonable,but there are still unreasonable phenomena,which require pharmacists to intervene and strengthen rational drug use.
