基层中西医结合医院药学门诊的建立及实践效果分析

中文摘要：目的介绍基层中西医结合医院药学门诊服务内容及流程，总结实践效果和存在的问题，提出改进建议，为提高药学门诊服务水平和质量提供参考。方法分别收集2022年11月～2023年10月克拉玛依市中西医结合医院事前审方系统的数据及2 804例患者用药咨询记录并进行汇总。其中，事前审方数据对人工干预情况、不合理处方原因、药品种类及事后处方点评合理率进行整理分析;用药咨询按照咨询内容、咨询者年龄、咨询药品种类、中西药联用情况及咨询者满意度进行归纳统计。结果药师复核合格处方38 009张，干预处方1 297张，干预后医生修改处方数1 220张;不合理处方原因主要为诊断与药物适应证不适宜(35。43%)、药品用法用量(32。51%)、特殊人群使用不当(16。74%)等;不合理处方品种以中成药居多(61。97%);事后处方点评合理率较前提高3。76%。咨询内容以药品用法用量为主，咨询者年龄以50～70岁居多，咨询药品种类主要为心血管药物、内分泌药物和消化系统药物等;中西药联用超过3种的有1 945例(69。37%);患者咨询满意度达到100%。结论基层中西医结合医院通过开设药学门诊，提供药学服务，可有效提高患者用药安全性和依从性，促进临床药学高质量发展。

外文摘要：Objective To introduce the contents and process of pharmacy outpatient in the grassroots integrated traditional Chinese and Western medicine hospital.The practical effects and existing problems were summarized and improvement suggestions were also proposed.The aims of those were provide a reference for improving service level and quality of pharmacy outpatient.Methods The data of prescription checking beforehand system and 2 804 cases of medication consultation records from Karamay Municipal Hospital of Integrated Traditional Chinese and Western Medicine during November 2022 to October 2023 were respectively collected.Manual intervention situation,reasons for unreasonable prescriptions,drug types and reasonability rate of post prescription review were summarized and analyzed through prescription checking beforehand data.Consultation contents,age and type of consultants,consulted medicine types,the number of combination of TCM and western medicine,satisfactory degree of consultants were organized and counted according to medication consultation records.Results The total of 38 009 reasonable prescriptions were reviewed by pharmacist,1 297 prescriptions were intervened and 1 220 prescriptions were modified through doctors after intervention.The main reasons for unreasonable prescriptions were inappropriate diagnosis and drug indications(35.43%),drug usage and dosage(32.51%)and improper use by special populations(16.74%).Most of the unreasonable prescriptions were TCM patent drug(61.97%),moreover,the rationality rate of post prescription review has increased 3.76%compared to before prescription review.The main consultation contents included usage and dosage,consultants mainly 50～70 years old,major consulted medicine types were cardiovascular system medicines,endocrine system medicines and digestive system medicines.In addition,types of TCM combined with western medicine more than 3 kinds have 1 945 cases(69.37%).100%patients were very satisfied with consultation for pharmacy outpatient.Conclusion The pharmacy care in the grassroots integrated traditional Chinese and Western medicine hospital can effectively improve patient medication safety and compliance,and promote the high-quality development of clinical pharmacy by providing pharmaceutical services through opening pharmacy outpatient.
