Evaluation of the effects of periodontal accelerated osteogenesis orthodontic treatment on periodontal and tooth root in patients with bimaxillary protrusion based on CBCT
Objective Based on oral and maxillofacial cone beam CT(CBCT),the effects of periodontal accelerated osteogenesis orthodontic(PAOO)on periodontal and tooth roots in patients with bimaxillary protrusion were evaluated.Methods Taking 40 patients with bimaxillary protrusion admitted to the stomatology department of a hospital from January 2021 to May 2023 as the research subjects,they were divided into the experimental group and the control group by using the random number table method,20 cases in each group.In both groups,the upper and lower first premolars were extracted,and routine correction was performed to align the dentition and smooth the Spee curve.The control group received implant anchorage,and the experimental group underwent PAOO on the basis of the control group,both groups were loaded with retracted anterior teeth immediately after surgery,and had regular follow-up visits to add force.Postoperative follow-up was conducted until 3 months after the extraction gap was closed.The labial gingival recession and keratinized gingival width in the lower anterior teeth area of the two groups were measured after Spee curve flattening and at the last follow-up,and the changes in periodontal soft tissue of the two groups were evaluated.CBCT was taken at the same time.The labial alveolar bone area,labial/lingual alveolar bone height,labial alveolar bone thickness and root length of the lower anterior teeth area were measured based on the CT images,and the changes in periodontal hard tissue and root length of the two groups were evaluated.Results At the last follow-up,the gingival recession and root length of the experimental group were compared with those of the control group,the differences were not statistically significant(P>0.05);the keratinized gingival width was greater than that of the control group,and the labial alveolar bone area,labial/lingual alveolar bone height,labial alveolar bone thickness were all higher than those of the control group,the differences were statistically significant(P<0.05).Conclusion The application of PAOO in the retraction of anterior teeth in patients with bimaxillary protrusion can effectively increase the keratinized gingiva width and alveolar bone mass in the lower anterior teeth area,which is beneficial to maintaining the height of alveolar bone height without increasing the risk of gingival recession and root resorption.
