脑转移瘤是成人最常见的中枢神经系统肿瘤之一,具有发展快、预后差的特点.随着放疗技术的不断发展和靶向治疗、免疫治疗等新型治疗方式的出现,脑转移瘤患者生存率有了很大提高.影像学上病灶的检出和评估对治疗方案的选择及患者预后至关重要.因此,现代医疗对影像学提出了更高的要求.常规影像学磁共振检查一直是诊断与评估脑转移瘤的金标准.随着影像技术的不断提高.MRI不只局限于从形态学评估脑转移瘤,磁共振波谱成像、脑灌注成像、扩散张量成像等功能成像更是从生物水平评估脑转移瘤状况.不仅如此,近年来涌现的放射组学与人工智能等新型技术更是有望将繁重的临床工作简单化,实现对病灶的检出及评估的智能化,大大提升工作效率及工作的准确率.本文从影像学对脑转移瘤的诊断和疗效评估进行综述.
Advances in Imaging Studies of Brain Metastases
Brain metastases are one of the most common central nervous system tumors in adults and are characterized by rapid development and poor prognosis.With the continuous development of radiotherapy technology and the emergence of new treatment modalities such as targeted therapy and immunotherapy,the survival rate of patients with brain metastases has been greatly improved.The detection and evaluation of lesions on imaging are crucial to the selection of treatment options and patient prognosis.Therefore,modern medical care has placed higher demands on imaging.Routine imaging MRI has been the gold standard for the diagnosis and evaluation of brain metastases.With the continuous improvement of imaging technology,MRI is not only limited to morphological assessment of brain metastases,but also functional imaging such as magnetic resonance spectroscopy,brain perfusion imaging,and diffusion tensor imaging to assess brain metastases at the biological level.In addition,the emergence of new technologies such as radiomics and artificial intelligence in recent years is expected to simplify the heavy clinical work and realize intelligent detection and evaluation of lesions,which greatly improves the efficiency and accuracy of work.This article reviews the diagnosis and efficacy assessment of brain metastases from imaging.
