Correlation between Transverse Sinus Stenosis and Transstenosis Pressure Gradient in Pulsatile Tinnitus
Objective To explore the relationship between morphological and hemodynamic factors of Transverse sinus stenosis(TSS)and Trans-stenostic pressure gradient(TPG)in pulsatile tinnitus(PT).Methods Data of unilateral PT patients admitted to Beijing Friendship Hospital,Capital Medical University from October 2018 to October 2023 were retrospectively analyzed.Temporal bone CTV and 4D Flow MR of venous sinus were performed and TSS of the symptomatic side were confirmed in all patients,and then venous sinus pressure was measured by DSA.The total length of TSS,TSS degree,trans-stenostic average blood flow velocity difference(Vavg,cm/s)and trans-stenostic maximum blood flow velocity difference(Vmax,cm/s)were measured on the symptomatic side,and the TPG of TSS was recorded.Pearson correlation analysis was used to evaluate the relationship between TSS length,TSS degree,trans-stenostic Vavg difference and trans-stenostic Vmax difference and TPG,and further multiple linear regression analysis was performed for significant independent variables.Results A total of 83 patients(12 males and 71 females)aged 39.0(49.0-30.0)years were included.On the symptomatic side,the Vavg difference,Vmax difference,TSS degree and length were linearly correlated with TPG,and Pearson correlation coefficients were 0.47(P<0.001),0.52(P<0.001),0.47(P<0.001)and 0.42(P<0.001),respectively.Multiple linear regression(stepwise)showed the Vmax difference((3=0.03,95%CI:0.02-0.04,P<0.001)and TSS degree(β=4.69,95%CI:0.80-8.57,P=0.02)and TSS length(β=0.14,95%CI:0.04-0.23,P=0.01)independently correlated with TPG(R=0.68,R2=0.46,P=0.02).Conclusion The trans-stenostic Vavg difference,Vmax difference,TSS degree and length are correlated with TPG,which can be used to predict the level of TPG.
