基于多模态MRI定量分析诊断宫颈癌宫旁浸润的研究

目的 探讨多模态MRI定量分析诊断宫颈癌宫旁浸润的价值.方法 回顾性分析2019年1月~2021年12月新疆维吾尔自治区人民医院收治的86例宫颈癌患者临床资料,根据病理结果分为宫旁浸润组31例与非宫旁浸润组55例.所有患者均接受MRI常规平扫及扩散加权成像(DWI)扫描、动态对比增强磁共振成像(DCE-MRI)扫描,获得感兴趣区表观弥散系数(ADC)值和对比剂容积转换常量(Ktrans)、速率常数(Kep)、血管外细胞间隙容积分数(Ve)值等多模态MRI定量参数.比较宫旁浸润组和非宫旁浸润组临床特征、ADC值和DCE-MRI定量参数;Pearson相关性分析宫颈癌宫旁浸润患者ADC值和Ktrans、Kep、Ve的关系;受试者工作特征曲线(ROC)分析ADC值和Ktrans、Kep、Ve对宫颈癌宫旁浸润的诊断效能.结果 宫旁浸润组与非宫旁浸润组肿瘤直径和病理分级的分布差异比较,差异有统计学意义(P<0.05);宫旁浸润组Ktrans、Kep和Ve均明显高于非宫旁浸润组(P<0.05);Pearson相关性显示,宫颈癌宫旁浸润患者ADC值与Ve值呈明显的正相关(P<0.05);ROC显示,Ktrans、Kep、Ve值诊断宫颈癌宫旁浸润的曲线下面积(AUC)分别为0.876、0.821 和0.864(P<0.05).结论 多模态MRI定量参数对宫颈癌宫旁浸润具有较好的诊断价值,可为宫颈癌宫旁浸润的影像诊断提供量化参考.
Diagnosis Value of Multimodal MRI Quantitative Analysis for Parametrial Infiltration in Cervical Cancer
Objective To explore the diagnosis value of multimodal MRI quantitative analysis for parametrial infiltration in cervical cancer.Methods A retrospective analysis was performed on the clinical data of 86 patients with cervical cancer admitted to People's Hospital of Xinjiang Uygur Autonomous Region between January 2019 and December 2021.According to pathological results,they were divided into parametrial infiltration group(31 cases)and non-infiltration group(55 cases).All underwent routine MRI plain scan,diffusion-weighted imaging(DWI)scan and dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)scan to obtain multimodal MRI quantitative parameters[apparent diffusion coefficient(ADC)in region of interest(ROI),volume transfer constant(Ktrans),rate constant(Kep),extravascular extracellular space fractional volume(Ve)].The clinical characteristics,ADC values and DCE-MRI quantitative parameters between the two groups were compared.The relationship between ADC value and Ktrans,Kep,Ve was analyzed by Pearson correlation analysis.The diagnostic efficiency of ADC value,Ktrans,Kep and Ve for parametrial infiltration was analyzed by receiver operating characteristic(ROC)curves.Results There were significant differences in tumor diameter and pathological grading between the two groups(P<0.05).Kep and Ve in parametrial infiltration group were significantly higher than those in non-infiltration group(P<0.05).Pearson correlation analysis showed that ADC value was significantly positively correlated with Ve(P<0.05).ROC curves analysis showed that area under curve(AUC)values of Ktrans,Kep and Ve in the diagnosis of parametrial infiltration were 0.876,0.821 and 0.864,respectively(P<0.05).Conclusion The multimodal MRI quantitative parameters have good diagnostic value for parametrial infiltration in cervical cancer,which can provide quantitative reference for imaging diagnosis of parametrial infiltration.
