多参数磁共振Ⅵ-RADS在膀胱癌病理分级及肌层浸润中的诊断价值

目的 评价基于膀胱影像报告和数据系统(Ⅵ-RADS)的多参数磁共振(mp-MRI)影像方法在膀胱尿路上皮癌(BUC)中的研究意义.方法 回顾性分析2018年5月到2022年2月安徽医科大学附属滁州医院有病理证实的48例(共49个病灶)行mp-MRI检查的BUC患者的资料.根据术后病理结果,将患者分为肌层浸润性膀胱癌(MIBC)和非肌层浸润性膀胱癌(NMIBC)组,低级别(LG)和高级别(HG)组,由两位影像科医生,模拟报告"双签"方式对这些病人图像按照Ⅵ-RADS的评分标准进行评分,再将Ⅵ-RADS的评分与病理分期、分级作对比,以此评价基于Ⅵ-RADS的mp-MRI影像方法在MIBC、HG中的诊断价值.应用受试者工作特征(ROC)曲线分析VI-RADS评分对2种BUC组织病理学特征的诊断效能.结果 两位评分者的Ⅵ-RADS评分一致性良好(符合率为79.60,kappa值=0.730,P<0.001).基于Ⅵ-RADS的mp-MRI影像方法在MIBC与NMIBC、LG与HG组间的差异有统计学意义(Z=-4.587、-2.871,P均<0.05).ROC曲线分析显示:当Ⅵ-RADS评分≥3.5时,诊断MIBC的曲线下面积(AUC)为0.915(95%CI 0.899-0.931),约登指数最大为0.665,敏感度66.67%,特异度99.86%;当评分≥2.5时,诊断HG的AUC为0.757(95%CI 0.730-0.783),约登指数最大为0.379,敏感度77.72%,特异度60.13%.Ⅵ-RADS评分对MIBC的诊断效能高于HG,差异有统计学意义(Z=-2.106,P<0.05).结论 基于Ⅵ-RADS的mp-MRI影像方法在MIBC和HG中具有较高的诊断效能,且对MIBC的诊断价值高于HG.
Diagnostic Value of Multi-parameter Magnetic Resonance Ⅵ-RADS in the Pathological Grading and Muscularity of Bladder Cancer
Objective To evaluate the significance of the multi-parameter magnetic resonance(mp-MRI)imaging method based on bladder imaging reporting and data system(Ⅵ-RADS)in bladder urothelial carcinoma(BUC).Methods The data of 48 BUC patients with pathological confirmation(a total of 49 lesions)in Chuzhou Hospital affiliated to Anhui Medical University from May 2018 to February 2022 were analyzed.According to the postoperative pathological results,the patients were divided into muscle layer invasive bladder cancer(MIBC)and non-muscle layer invasive bladder cancer(NMIBC)groups,low grade(LG)and high grade(HG)groups.The two imaging physicians reported the images of these patients according to the Ⅵ-RADS,and then the score of Ⅵ-RADS was compared with the pathological stage and grade to evaluate the diagnostic value of mp-MRI imaging method based onⅥ-RADS in MIBC and HG.Subject operating characteristic(ROC)curves were used to analyze the diagnostic efficacy of Ⅵ-RADS scores on the histopathological features of two BUC.Results Ⅵ-RADS agreement between the rat(79.60,kappa value=0.730,P<0.001).The mp-MRI imaging method based on Ⅵ-RADS showed a significant difference between MIBC and NMIBC and LG and HG groups(Z=-4.587 and-2.871,P<0.05).ROC curve analysis showed that the area under the curve(AUC)for diagnosed MIBC was 0.915(95%CI 0.899-90.931)for Ⅵ-RADS 3.5,yoden index 0.665,sensitivity 66.67%,specificity 99.86%,0.757(95%CI 0.730-0.783),0.379,sensitivity 77.72%,and specificity 60.13%.The Ⅵ-RADS score was higher than HG and statistically significant(Z=-2.106,P<0.05).Conclusion The Ⅵ-RADS based mp-MRI imaging method has high diagnostic efficacy in MIBC and HG and higher diagnostic value for MIBC than HG.
