新生儿缺氧缺血性脑病磁共振检查特点及与脑损伤的相关性分析

目的 探讨新生儿缺氧缺血性脑病(HIE)磁共振(MRI)检查特点及与脑损伤的相关性.方法 回顾性选择2018年3月到2023年3月我院收治的115例足月HIE患儿,依据HIE临床分度分为轻度组(n=41)、中度组(n=53)重度组(n=21).比较三组患者的一般资料、MRI影像学评分、表观弥散系数(ADC);采用Pearson相关性检验分析各部位MRI影像学评分与脑损伤的相关性,通过受试者工作特征(ROC)曲线验证各表观弥散系数(ADC)诊断脑损伤风险的预测价值.结果 三组在性别、胎龄、出生体质量、分娩方式、母体妊娠期合并症、围产期情况、灰白质分界模糊、深部白质侧脑室壁、皮层及以下点状和T1WI高信号的临床资料差异无统计学意义(P>0.05).三组患儿双侧丘脑-基底节区不对称性、脑内静脉扩张、颅内出血差异有统计学意义(P<0.05).三组MRI影像学评分的分水岭区、基底节/丘脑、内囊后肢、脑干、综合评分均随着脑损伤程度加深而增高,差异有统计学意义(P<0.05).Pearson分析结果显示,分水岭区、基底节/丘脑、内囊后肢、脑干和综合评分与脑损伤程度均呈正相关性(P<0.05).三组患儿腹外侧丘脑ADC值均随着10th ADC~90th ADC逐渐升高.重度组60th ADC~90th ADC、ADCmax、ADCmin、ADCmean均大于中度组和轻度组,中度组患儿60th ADC~90th ADC、ADCmax、ADCmin、ADCmean均大于轻度组,差异有统计学意义(P<0.05).ROC分析结果显示,90th ADC的AUC最大0.895(95%CI:0.762~0.965),C-index=0.876(95%CI:0.781~0.997),以848.19为最佳截点,灵敏度为85.78%,特异度为90.36%.MRI影像综合评分联合90th ADC值诊断HIE患儿脑损伤程度的灵敏度(89.77%)高于二者分别单独诊断的灵敏度(70.71%和73.28%).结论 HIE患儿双侧丘脑-基底节区不对称性、脑内静脉扩张、颅内出血与脑损伤程度有关,MRI影像学评分与脑损伤程度呈正相关性,MRI影像综合评分联合90th ADC值诊断HIE患儿脑损伤程度灵敏度89.77%.
The Characteristics of Magnetic Resonance Examination in Neonatal Hypoxic-ischemic Encephalopathy and Its Correlation with Brain Injury
Objective To investigate the characteristics of magnetic resonance imaging(MRI)in neonatal hypoxic-ischemic encephalopathy(HIE)and its correlation with brain injury.Methods 115 full-term HIE children admitted to our hospital from March 2018 to March 2023 were selected retrospectively.According to HIE clinical classification,the patients were divided into mild group(n=41),moderate group(n=53)and severe group(n=21).The general information,MRI imaging score and apparent diffusion coefficient(ADC)of the three groups were compared.Pearson correlation test was used to analyze the correlation between MRI imaging scores and brain injury.The predictive value of ADC in diagnosing the risk of brain injury was verified by the ROC curve.Results There was no significant difference among the three groups in the clinical data of gender,gestational age,birth weight,delivery mode,maternal pregnancy complications,perinatal conditions,blurry boundary of gray white matter,deep white matter lateral ventricular wall,cortex and below dots and T1WI high signal(P>0.05).There were significant differences among the three groups in bilateral thalamus basal ganglia asymmetry,cerebral venous dilatation and intracranial hemorrhage(P<0.05).The watershed area,basal ganglia/thalamus,posterior limb of internal capsule,brain stem and comprehensive score of MRI imaging in the three groups increased with the deepening of brain injury.The difference was statistically significant(P<0.05).Pearson analysis showed that watershed area,basal ganglia/thalamus,posterior limb of internal capsule,brain stem and comprehensive score were positively correlated with the degree of brain injury(P<0.05).ADC value of the three groups increased gradually with the 10th ADC~90th ADC.The 60th ADC~90th ADC,ADCmax,ADCmin,ADC mean in severe group were higher than those in moderate group and mild group.The 60th ADC~90th ADC,ADCmax,ADCmin and ADCmean in the moderate group were higher than those in the mild group.The difference was statistically significant(P<0.05).ROC analysis results show that the AUC of the 90th ADC is 0.895(95%CI:0.762~0.965).C-index=0.876(95%CI:0.781~0.997).The best cutoff point is 848.19.The sensitivity was 85.78%,and the specificity was 90.36%.The sensitivity of MRI combined with 90th ADC value in diagnosing the degree of brain injury in HIE children(89.77%)was higher than that of the two alone(70.71%and 73.28%).Conclusion The asymmetry of bilateral thalamus basal ganglia,the expansion of cerebral veins and intracranial hemorrhage in HIE children are related to the degree of brain injury.MRI imaging score was positively correlated with the degree of brain injury.The sensitivity of MRI combined with 90th ADC value in diagnosing the degree of brain injury in HIE children was 89.77%.
