MRI检查在小儿急性化脓性骨髓炎早期筛查中的作用

摘要
目的 研究小儿急性化脓性骨髓炎早期筛查中应用磁共振成像(MRI)检查的作用.方法 选取我院2020年8月-2023年1月收治的62例急性化脓性骨髓炎患儿为研究对象,所有患儿均行X线、MRI检查,比较比较患儿不同发病时间X线、MRI检查图像表现及不同检查方式骨髓异常、软组织病变检出情况.结果 患儿发病1周内X线检查均未见异常图像表现,发病8-14d可见软组织肿胀,肌肉骨膜间距、皮下脂肪层厚度改变;而MRI检查在发病3-4d即显示STIR高信号、均匀低等T1信号,发病5-7d可见混杂信号及骨膜水肿表现,发病8-14d可见等低T1WI信号、等高T2WI信号及骨膜下脓肿表现.MRI检查骨髓异常、软组织病变检出率分别为100.00%、90.32%,与X线检查的19.35%、48.39%相比均更高(P<0.05).结论 MRI检查能够早期诊断小儿急性化脓性骨髓炎,应用价值较高.
Abstract
Objective To study the role of magnetic resonance imaging(MRI)in early screening of acute suppurative osteomyelitis in children.Methods 62 children with acute suppurative osteomyelitis admitted to our hospital from August 2020 to January 2023 were selected as the study subjects.All children underwent X-ray and MRI examinations,and the X-ray and MRI imaging manifestations at different onset times,as well as the detection of bone marrow abnormalities and soft tissue lesions using different examination methods,were compared.Results Within one week of onset,there were no abnormal imaging findings on the X-ray examination of the child.From 8 to 14 days after onset,soft tissue swelling,changes in muscle periosteal spacing,and subcutaneous fat layer thickness were observed;MRI examination showed high STIR signal and uniform low T1 signal within 3-4 days of onset,mixed signal and periosteal edema within 5-7 days of onset,and low T1WI signal and high T2WI signal and subperiosteal abscess within 8-14 days of onset.The detection rates of bone marrow abnormalities and soft tissue lesions in MRI examination were 100.00%and 90.32%,respectively,which were higher than 19.35%and 48.39%in X-ray examination(P<0.05).Conclusion MRI examination can early diagnose acute suppurative osteomyelitis in children and has high application value.
