目的 探究动态对比增强磁共振成像技术(dynamic contast enhancement-magnetic resonance imaging,DCE-MRI)在宫颈癌分期诊断中的应用价值.方法 研究对象为我院2019年1月~2022年1月期间收治的经病理学检查确诊为宫颈癌的患者,经纳入排除后,共得240例.患者均接受手术治疗,治疗前及治疗末均行MRI影像学检查和DCE-MRI检查,同时,治疗前择期将组织病理学检查(包括最新WHO病理学分型)纳入分期系统中,确保本研究肿瘤大小更精准.观察并比较DCE-MRI与病理学检查对宫颈癌分期诊断准确性,以及DCE-MRI对患者预后情况的指导意义.结果 以组织病理学检查结果为"金标准",Ⅰ~Ⅱ期患者共诊断出157例,Ⅲ~Ⅳ期共诊断出83例.DCE-MRI的病例分期诊断准确率为96.67%(232/240),8例未检出,分期诊断结果与组织病理学结果相比无明显差异(P>0.05)o DCE-MRI评估Ⅲ~Ⅳ期宫颈癌分期的灵敏度为91.57%(76/83),特异度为99.36%(156/157).观察不同分化程度及不同分期的患者DCE-MRI的药代动力学参数,癌细胞高分化程度患者定量参数容量转移参数(volume transfei constant,Ktrans)、速率常数(rate constant,Kep)、细胞外血管外间隙体积百分数(extravascular extracellular space leakage space,Ve)水平低于低分化和中分化程度患者(P<0.05);Ⅲ~Ⅳ期患者Ktrans、kep、Ve水平高于Ⅰ~Ⅱ期患者(P<0.05).随访过程中,出院后复发患者80例(33.33%),复发患者Ktrans、kep、Ve水平高于未复发患者(P<0.05).结论 DCE-MRI对宫颈癌患者分期诊断准确性高,可协助临床预测患者预后,有较高的应用价值.
Dynamic Comparison of DCE-MRI for Staging and Diagnosis of Cervical Cancer
Objective To explore the application value of dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)in the staging diagnosis of cervical cancer.Methods The study subjects were 240 patients diagnosed with cervical cancer through pathological examination admitted to our hospital from January 2019 to January 2022.All patients received surgical treatment,MRI imaging examination and DCE-MRI examination were performed before and after treatment.At the same time,pathological examinations(including the latest WHO pathological classification)were included in the staging system at a selected time before treatment,ensuring more accurate tumor size in this study.The accuracy of DCE-MRI and pathological examination in staging diagnosis of cervical cancer was observed and compared,as well as the guiding significance of DCE-MRI for patient prognosis.Results Using histopathological examination results as the gold standard,a total of 157 cases were diagnosed in stage Ⅰ to stage Ⅱpatients,and 83 cases were diagnosed in stage Ⅲ to stage Ⅳ.The accuracy of stage diagnosis in DCE-MRI cases was 96.67%(232/240),and 8 cases were not detected.There was no significant difference between stage diagnosis results and histopathological results(P>0.05).The sensitivity and specificity of DCE-MRI in evaluating the staging of stage Ⅲ-Ⅳ cervical cancer were 91.57%(76/83)and 99.36%(156/157),respectively.Observing the pharmacokinetic parameters of DCE-MRI in patients with different degrees of differentiation and stages,the quantitative parameters of volume transfer constant(Ktrans),rate constant(Kep),and extracellular extracellular space leakage space(Ve)in patients with high differentiation of cancer cells were lower than those in patients with low and medium differentiation(P<0.05);The levels of Ktrans,kep,and Ve in patients with stage Ⅲ-Ⅳ were higher than those in patients with stage Ⅰ-Ⅱ(P<0.05).During the follow-up process,80 patients(33.33%)relapsed after discharge,and the levels of Ktrans,kep,and Ve in relapsed patients were higher than those in non relapsed patients(P<0.05).Conclusion DCE-MRI has high accuracy in staging diagnosis of cervical cancer patients,can assist in clinical prediction of patient prognosis,and has high application value.
