新疆石河子市女性乳腺癌疾病负担研究

摘要
目的:分析新疆石河子市女性乳腺癌疾病分布及死亡状况,评价女性乳腺癌的疾病负担,为制定科学合理的防治措施提供依据.方法:利用石河子市2010 ～2011年女性乳腺癌死亡资料,统计分析女性乳腺癌的发病率、标化发病率、死亡率、标化死亡率、潜在减寿年(PYLL)、伤残调整寿命年(DALY)、死亡寿命损失年(YLL)及伤残寿命损失年(YLD),评价女性乳腺癌的疾病负担.结果:2010 ～2011年新疆石河子市女性乳腺癌死亡人数为64人,死亡率为11.21/10万,中标率(1982年中国人口构成)为5.72/10万,高于中国平均水平(4.93/10万).2010 ～2011年石河子市女性乳腺癌PYLL为952.5人年,PYLL率为0.18‰.2010 ～2011年石河子市女性乳腺癌损失517.22个DALYs,占女性恶性肿瘤DALY损失的7.35％,平均每千人损失1.82个DALY,其中因早亡导致的YLL为510.98,因伤残导致的YLD为6.25.2010 ～2011年石河子市女性乳腺癌DALY主要分布在40 ～50岁年龄组.结论:女性乳腺癌严重危害石河子市女性生命健康,对中老年女性健康危害尤为严重.应将40 ～50岁女性作为干预重点,积极采取防治措施,降低石河子市女性乳腺癌的疾病负担.
Abstract
Objective:To analyze the distribution and mortality of female breast cancer in Shihezi,Xinjiang,evaluate disease burden of female breast cancer,and provide a basis for formulating scientific and rational prevention measures for female breast cancer.Methods:The data of death of female breast cancer in Shihezi from 2010 to 2011 were collected to statistically analyze the incidence rate,standardized incidence rate,mortality rate,standardized mortality rate,years of potential life lost (YPLL),disability adjusted life year (DALY),years of life lost (YLL) and years lived with disability (YLD),and evaluate disease burden of female breast cancer.Results:From 2010 to 2011,64 women died of breast cancer in Shihezi,the mortality rate was 11.21/100 000,ASR China (China population composition in 1982) was 5.72/100 000,which was higher than national average level (4.93/100 000).PYLL of breast cancer in Shihezi from 2010 to 2011 was 952.5,PYLL rate was 0.18‰.DALY was 517.22 years,accounting for 7.35％ of the malignant tumors,1.82 DALY per thousand on average,YLL induced by premature death was 510.98,YLD induced by impairments was 6.25.DALY mainly distributed in 40-50-year-old group.Conclusion:Female breast cancer severely impair female life health in Shihezi,especially for middle-aged and old women.Intervention among 40-50-year-old women should be emphasized,prevention and treatment should be conducted actively to reduce disease burden of female breast cancer in Shihezi.
