Analysis on the value of Sonazoid ultrasound contrast,vimentin,glucose transporter-1,calmodulin,and P63 protein expressions in sentinel lymph node metastasis of breast cancer
Objective To explore the value of Sonazoid ultrasound contrast,vimentin,glucose transporter-1(GLUT-1),calmodulin,and P63 protein expressions in sentinel lymph node metastasis of breast cancer.Methods From March 2021 to December 2022,98 patients with breast cancer treated in Yiwu Central Hospital were selected as breast cancer group,50 patients with breast benign hyperplasia were se-lected as control group.The patients in breast cancer group underwent Sonazoid ultrasound contrast before operation,focal property was eval-uated,the metastasis of sentinel lymph node was determined,then the results were compared with histopathological examination results after operation,the diagnostic value was analyzed.Conventional ultrasound was used to scan size of sentinel lymph node,transverse diameter and longitudinal diameter of sentinel lymph nodes were measured,aspect ratio was calculated.Immunohistochemical method was used to detect the expressions of vimentin,GLUT-1,calmodulin,and P63 protein in breast tissues of patients in breast cancer group and control group.Results Among 98 breast cancer patients,Sonazoid ultrasound contrast detected 122 sentinel lymph nodes,including 65 enhancement mode A lymph nodes(53.28%),49 enhancement mode B lymph nodes(40.16%),and 8 enhancement mode C lymph nodes(6.56%).The sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of Sonazoid ultrasound contrast in diagnosis of sen-tinel lymph node metastasis were 94.74%(36/38),75.00%(63/84),81.15%(99/122),63.16%(36/57),and 96.92%(63/65),respectively.The transverse diameter and longitudinal diameter of sentinel lymph node in metastasis group were significantly longer than those in non-metastasis group(P<0.05).The positive expression rates of vimentin,GLUT-1,and P63 protein in breast cancer group were 76.53%,62.24%,and 79.59%,respectively,which were significantly higher than those in control group(16.00%,12.00%,6.00%),the positive expression rate of calmodulin in breast cancer group was 14.29%,which was significantly lower than that in control group(90.00%),there were statistically significant differences(all P<0.05).In breast cancer group,the positive expression rates of vimentin,GLUT-1,and P63 protein in metastasis group were 89.66%,86.21%,and 93.10%,respectively,which were significantly higher than those in non-metastasis group(71.01%,52.17%,73.91%),the positive expression rate of calmodulin in metastasis group was 6.90%,which was significantly lower than that in non-metastasis group(17.39%),there were statistically significant differences(all P<0.05).Receiver operating characteristic(ROC)curve showed that transverse diameter,longitudinal diameter,vimentin,GLUT-1,calmodulin,and P63 protein had high value in diagnosis of sentinel lymph node metastasis of breast cancer(P<0.05),the diagnostic value of combined detection was the highest,the area under ROC curve was 0.975.Conclusion Sonazoid ultrasound contrast before operation has high diagnostic value for sentinel lymph node metastasis of breast cancer,combined detection of transverse diameter,longitudinal diameter,vimentin,GLUT-1,calmodulin,and P63 protein can significant-ly improve the diagnostic efficacy for sentinel lymph node metastasis of breast cancer.
