摘要
目的 编制适合我国居民的就医健康素养量表并检验其信效度.方法 参考国内外已有的健康素养测量工具,结合我国医疗机构的就医流程现状,设计适合我国居民的健康素养量表.采用线上调查的方法,对初测的384个样本进行探索性因子分析,对实测的2 773个样本进行验证性因子分析,间隔2周另选取20名居民进行重测.选取受教育水平作为校标.结果 探索性因子分析结果显示,量表共23个条目,包含4个公因子,累计方差解释率为63.930%,各条目的因子负荷值在0.527~0.903之间;验证性因子分析结果显示,二阶因子模型拟合良好(x2/df=10.697,RMSEA=0.059,CFI=0.952,TLI=0.945,SRMR=0.065),各因子负荷在0.464~0.936之间.就医健康素养得分与调查对象的受教育水平呈正相关(ρ=0.258,P<0.001).总量表的Cronbach's α系数为0.923,各维度的Cronbach's α系数为0.712~0.949;总量表的重测信度为0.879,各维度的重测信度为0.703~0.802.结论 所编制的居民就医健康素养量信效度较好,可用于评估居民在医疗机构内临床环境中的健康素养.
Abstract
Objective To develop a medical health literacy scale suitable for Chinese residents and test its reliability and validity.Methods By referring to the existing health literacy scales both domestically and internationally,and com-bining with the the current medical process of medical institutions in China,a health literacy scale suitable for residents in China was designed.Using online survey method,exploratory factor analysis was conducted on 384 samples from the pre-investigation;confirmatory factor analysis was conducted on 2 773 samples from the actual test.And after a 2-week interval,another 20 residents were selected for retesting.The criterion validity was examined with education level.Results The results of exploratory factor analysis showed that the scale had a total of 23 items,including 4 common factors,with a cumulative variance explanatory rate of 63.930%.The factor loading of items ranged between 0.527-0.903.The confirmatory factor analysis verified that the fitting degree of second order model was acceptable(x2/df=10.697,RMSEA=0.059,CFI=0.952,TLI=0.945,SRMR=0.065).The factor loading of items ranged from 0.464 to 0.936.The score of health literacy was positively correlated with the educational level(ρ=0.258,P<0.001).The Cronbach's alpha coefficients of were 0.923 for the total scale and 0.712-0.949 for each dimension.The total test-retest reliability was 0.879,and each dimension ranged 0.703-0.802.Conclusion The health literacy scale has good evidence for validity and reliability,which could be used to measure the health literacy of residents in the clinical environment of hospitals.
