摘要
目的 探究老年2型糖尿病患者(Type 2 Diabetes Mellitus,T2DM)营养不良的影响因素,并构建风险预测模型.方法 采用便利抽样法,于2023年1-10月抽取杭州市城区325例老年T2DM患者作为调查对象.采用一般资料调查问卷、微型营养评估量表进行调查,采用二元logistic回归模型分析营养不良的影响因素,建立风险预测模型并绘制受试者工作特征曲线.结果 本研究共检出营养不良患者79例,发生率为24.31%.年龄≥70岁、HbA1c≥7%、多重用药、合并症≥3种是调查对象营养不良的危险因素(P<0.05);BMI≥18.5 kg/m2、白蛋白≥35 g/L、规律运动是调查对象营养不良的保护因素(P<0.05).风险预测模型受试者工作特征曲线下面积为0.891,特异度为0.843,灵敏度为0.796,模型准确性尚可.结论 高龄、低BMI、低白蛋白、高HbA1c、多重用药、多种合并症易引发老年T2DM患者营养不良.预测模型可科学、直观地识别存在营养不良风险的老年T2DM患者.
Abstract
Objective To explore the influencing factors of malnutrition in elderly type 2 diabetes mellitus(T2DM)patients and build a risk prediction model.Methods Using convenience sampling method,325 elderly T2DM patients were selected from January to October 2023,and general data questionnaire and micro nutrition assessment scale were used for investigate,and binary logistic regression was used to analyze the influencing factors of malnutrition.The risk prediction model was established and the calibration curve was drew.Results A total of 79 malnutrition patients were detected,with an incidence of 24.31%.Age ≥70 years old,HbA1c≥7%,multiple drug use and comorbidities≥3 were risk factors for malnutrition in the surveyed subjects(P<0.05);BMI≥18.5 kg/m2,Albumin levels ≥35 g/L and regular exercise were protective factors for malnutrition in the surveyed subjects(P<0.05).The area under the model ROC curve was 0.891,the specificity was 0.843,the sensitivity was 0.796,and the model accuracy was acceptable.Conclusion Advanced age,low BMI,low albumin,high HbA1c,multiple medications,and multiple comorbidities can easily lead to malnutrition in elderly T2DM patients.The predictive model can scientifically and intuitively identify elderly T2DM patients at risk of malnutrition.
