Application of six sigma management on pre-analytical quality control of blood culture samples
Objective To improve the quality of clinical blood culture samples delivery by applying six sigma management methods. Methods A tertiary general hospital started to use six sigma management to intervene in the clinical blood culture samples delivery process in August 2022,using its own before and after control test, setting August 2021-February 2022 as the pre-intervention phase and August 2022-February 2023 as the post-intervention phase,to investigate and compare the composition ratio of pre-intervention blood culture delivery methods. The blood culture positivity rate, blood culture contamination rate, and blood culture samples pass rate, composition ratio of blood culture delivery methods, blood culture positivity rate, blood culture contamination rate and blood culture samples pass rate were compared between pre-intervention and post-intervention.Results The pre-intervention and post-intervention blood culture delivery samples were 2 577 and 2 554 cases, respectively. The rates of single and ≥ double sets of blood culture for testing and positive blood culture before intervention were 87.66%,11.49%and 9.24%,respectively.After intervention, the rates were 28.93%,69.97%and 12.88%,respectively,and the differences were statistically significant(P<0.001).The rate of single bottle of blood culture delivery, blood culture contamination rate, blood culture samples qualified rate before the intervention were 0.85%,0.74%,99.92%,respectively.After intervention, the rates were 1.10%,1.21%and 100.00%,respectively,the difference were not statistically significant(P>0.05).Conclusion Six sigma management standardized the delivery and sampling methods of clinical blood culture samples, and further improved the quality of clinical blood culture specimen delivery.
