Serum ferritin level in scrub typhus complicated with liver injury and its application value in clinical examination and diagnosis
Objective To explore the clinical value of serum ferritin(SF)in diagnosing scrub typhus complicated with liver injury and offer a scientific foundation for early detection and treatment.Methods A retrospective analysis of 84 inpatients with scrub typhus at Suqian First Hospital of Nanjing Medical University from October 2021 to November 2023 were selected as the research objects,and they were categorized into two groups based on the presence or ab-sence of liver dysfunction,44 cases in liver injury group and 40 in non-liver injury group.Then the differences of indicators including white blood cell count(WBC),platelet count(PLT),SF,alanine transaminase(ALT)and aspar-tate transaminase(AST)between these two groups were analyzed.The risk factors for liver injury in scrub typhus were examined,and the application value of PLT and SF levels in the detection and diagnosis of scrub typhus was evaluated through the area under the ROC curve(AUC).Results In the liver injury group,the median levels of SF,ALT and AST were higher than non-liver injury group[931.71(429.05,1555.52)vs 377.65(239.75,677.24),105.60(70.25,163.25)vs 39.1(25.18,47.38),95.35(70.23,129.00)vs 42.90(26.98,55.23)],respectively,with statistically sig-nificant differences(P<0.05),mean level of PLT was lower than non-liver injury group[(155.91±55.25)vs(186.68±67.99)],also showed statistically significant differences(P<0.05).Binary logistic regression analysis revealed that ele-vated SF and declined PLT levels may be the independent risk factors for scrub typhus complicated with liver injury(P<0.05).The AUC for SF was 0.746,and for PLT,it was 0.641.Conclusion Platelet and serum ferritin might play a predictive role in patients with scrub typhus presenting liver injury and serve as auxiliary indicators for the diag-nosis of scrub typhus complicated with liver injury.
