Analysis of Mycobacterium tuberculosis infection and risk factors in maintenance hemodialysis patients with end-stage renal disease
Objective To analyze Mycobacterium tuberculosis infection and its risk factors in maintenance hemodialysis patients with end-stage renal disease.Methods A total of 120 patients with end-stage renal disease who were treated with maintenance hemodialysis from January 2021 to December 2022 in Jingzhou Central Hospital of Hubei Province were selected,and they were divided into two groups with or without Mycobacterium tuberculosis infection.Mycobacterium tuberculosis was detected positive in group A(n=25),Mycobacterium tuberculosis was negative in group B(n=95).The influencing factors of Mycobacterium tuberculosis infection were analyzed.Results Among 120 patients with end-stage renal disease who received maintenance hemodialysis,Mycobacterium tuberculosis was detected positive in 25 cases,and 18 of them had a fever history lasting more than 1 month.The age above 60 years old,smoking history,antibiotic use(>2 kinds),tuberculosis exposure history,neutrophil level,CRP level and no BCG vaccination history in group A were all higher than those of group B(P<0.05).Multivariate Logistic regression analysis showed that age>60 years old,history of smoking,neutrophil ≥6.3×109/L,history of tuberculosis exposure,use of more than 2 antibiotics,and no history of BCG vaccination were the risk factors for Mycobacterium tuberculosis positive in patients with end-stage renal disease treated by maintenance hemodialysis(P<0.05).Conclusion Age,smoking history,neutrophils,tuberculosis exposure history,antibiotic use and BCG vaccination history were the main factors influencing the occurrence of Mycobacterium tuberculosis infection in patients with end-stage renal disease treated by maintenance hemodialysis.
