Analysis of factors influencing the condition of respiratory syncytial virus capillary bronchiolitis in infants and young children
Objective To analyze the influencing factors related to the condition of respiratory syncytial virus(RSV)capillary bronchiolitis in infants and young children,and to provide a basis for clinical prevention and treatment.Methods Seventy-eight cases of RSV capillary bronchiolitis admitted to Linquan County People's Hospital from February 2020 to May 2023 were selected to be included in the observation group,which was divided into mild(32 cases),moderate(28 cases)and severe group(18 cases)according to their conditions,and 65 healthy infants and young children who had a physical examination during the same period were selected to be included in the control group.The general data of the two groups were compared.Periostin(POSTN),eosinophil cationic protein(ECP),25-hydroxyvitamin D3[25(OH)D3],interleukin 10(IL-10),high mobility group protein B1(HMGB1),leukotriene C4 synthase(LTC4S)A-444C genotype and allele distribution of the children in the different groups were compared.Results There was no statistically significant difference in gender,age,BMI,whether preterm birth,and birth weight be-tween the two groups(P>0.05).The proportion of artificial feeding mode(35.90%),POSTN(498.63±32.66),ECP(18.54±5.67),IL-10(13.05±5.24)and HMGB1(0.41±0.05)in the observation group was higher than the control group(P<0.05),and 25(OH)D3(47.55±7.33)was lower than the control group(P<0.05).POSTN,ECP,IL-10 and HMGB1 of the three groups were in the order of mild group<moderate group<severe group(P<0.05)from low to high,while 25(OH)D3 was in the order of severe group(4327±7.02)<moderate group(52.47±6.63)<mild group(70.24±5.36)(P<0.05).The proportion of AA genotype in the mild group(53.13%)was lower than that in the moderate group(85.71%)and severe group(100.00%)(P<0.05),while the difference between the moderate group and the severe group was not statistically significant(P>0.05).The distribution of A allele in the mild group(70.31%)was lower than that in the moderate group(89.29%)and severe group(97.22%)(P<0.05),while the difference between the moderate and severe groups was not statistically significant(P>0.05).Logistic regression analysis revealed that artificial feed-ing,POSTN,ECP,25(OH)D3,IL-10,HMGB1,AA genotype and A allele distribution were the influencing factors for exacerbation of RSV in children with capillary bronchiolitis(P<0.05).Conclusion Artificial feeding,POSTN,ECP,25(OH)D3,IL-10,HMGB1,AA genotype and A allele distribution were associated with disease severity in children with RSV capillary bronchiolitis.
