Therapeutic effect of double fixation with thread anchor through bone tension band and high-strength thread hoop on patellar fracture
Objective To observe the clinical efficacy of tension band wiring and high-strength wire cerclage for double fixation in the treatment of patellar fractures,analyzing the safety and feasibility of this surgical approach for treating patellar fractures.Methods The data from 20 patients diagnosed as patellar fracture were retrospectively analyzed.All were treated with dual fixation using tension band wiring and high-strength wire cerclage in the outpatient and emergency department of Gansu Provincial Hospital from April 1,2021,to December 1,2021.Among them,12 were male and 8 were female,with an average age of(46.7±11.8)years.All patients had fresh,isolated patellar fractures without other associated injuries or serious internal medical conditions.They underwent dual fixation treatment with tension band wiring and high-strength wire cerclage in our hospital.Postoperative adverse reactions were documented,and routine knee joint anteroposterior and lateral X-ray images were taken postoperatively and 12 weeks postoperatively.Fracture healing time,postoperative complications,and the need for secondary surgery were recorded.Assessment using the Lysholm score and the Hospital for Special Surgery Knee Score(HSS score)was applied 12 weeks postoperatively.Results All patients achieved satisfactory fracture healing with no evidence of loosening or refracture of internal fixation.Lysholm scale 12 weeks postoperatively:excellent,12 cases;good,8 cases;average score(94.95±3.87).HSS scale 12 weeks postoperatively:excellent,19 cases;good,1 case;average score(90.80±3.87).Conclusions The dual fixation treatment of patellar fractures using tension band wiring and high-strength wire cerclage achieves favorable therapeutic outcomes.It enables accurate fracture reduction,promotes fracture healing(particularly in comminuted fracture segments),alleviates postoperative pain,and obviates the need for secondary surgery for implant removal.
